光学相干断层扫描对糖尿病性白内障术后黄斑厚度变化的临床观察

【摘要】:目的: 应用光学相干断层扫描仪(Optical Coherence Tomography OCT)观察糖尿病性白内障患者接受白内障超声乳化联合人工晶体植入术后黄斑中心凹视网膜厚度(Foveal retinal thickness, FRT)变化规律和其与视力的相关性,以及术后加用普拉洛芬眼液对其的影响。 方法: 选择2010年2月至2011年1月单纯年龄相关性白内障患者(62眼)及糖尿病性白内障患者(60眼),均于中南大学湘雅二医院眼科接受白内障超声乳化联合折叠式人工晶体植入术,及医院体检的正常人(30眼)共152眼。分为4组:A组:单纯年龄相关性白内障组31眼;B组:糖尿病性白内障组32眼,其中B1组糖尿病病程≤10年10眼,B2组糖尿病病程10年22眼;C组:术后加用普拉洛芬眼液组59眼,其中C1组无糖尿病31眼,C2组糖尿病病程≤10年11眼,C3组糖尿病病程10年17眼;D组:正常组30眼。OCT检测A、B、C各组术前及术后1周、术后1月、术后3月黄斑中心凹视网膜厚度及D组的黄斑中心凹视网膜厚度,比较各组及各组间手术前后黄斑中心凹视网膜厚度的差别,分析术后视力与黄斑中心凹视网膜厚度的关系及普拉洛芬眼液对黄斑水肿的防治作用。 结果: 1.各组术前FRT分别为:A组152.00±10.14um,B1组149.10±15.96um,B2组155.95±17.18um,C1组158.27±16.33um,C2组160.09±19.62um,C3组157.47±18.35um,D组158.55±12.04um,各组比较均无显著性差异(P0.05)。 2.A组术前与术后1周,1月,3月的FRT分别为:152.00±10.14um,165.2±23.93um、165.5±16.37um、157.2±20.48um。与术前相比,术后各时间点FRT无显著性差异(P0.05)。 3.B组术前与术后1周,1月,3月的FRT分别为:B1组:149.10±15.96um、193.8±31.73um、184.7±22.37um、158.5±15.13um; B2组:155.95±17.18um、212.4±29.09um、206.6±28.56um、171.4±16.37um.与术前相比,B1组在术后1周及1月FRT增厚,差异有统计学意义(P0.05);在术后3月厚度无显著性差异(P0.05)。B2组术后1周、1月及3月时FRT增厚,差异均有统计学意义(P0.05)。 4.C组术前与术后1周,1月,3月的FRT分别为:C1组:158.27±16.33um、169.1±22.83 um、170.1±20.02 um、158.8±19.36 um; C2组:160.09±19.62um、182.6±22.88 um、182.6±15.81um、165.9±19.85um; C3组:157.47±18.35um、186.6±35.78 um、182.9±26.63um、164.2±21.36um.与术前相比,C1组术后各时间点FRT无显著性差异(P0.05)。C2组及C3组术后1周及1月时FRT增厚,差异有统计学意义(P0.05),术后3月时FRT无显著性差异(P0.05)。 5.A组与B1组、B2组、C1组术后比较FRT增厚有统计学意义的是:B1组术后1周及术后1月;B2组术后1周,术后1月及术后3月(P0.05);余FRT无显著性差异(P0.05) 6.B1组与B2组术后比较:B2组术后1月FRT较B1组明显增厚,两组间有显著性差异(P0.05);术后1周及术后3月两组间无显著性差异(P0.05)。 7.B1组与C2组术后比较:B1组术后各观测时间点FRT与C2组无显著性差异(P0.05)。 8.B2组与C3组术后比较:B2组术后1周及术后1月FRT较C3组明显增厚,两组间有显著性差异(P0.05)。术后3月两组间无显著性差异(P0.05)。 9.各组术眼FRT与视力的相关性:呈负相关的有:B1组术后1月时(r=-0.764,P0.05);B2组术后1月及3月时(r=-0.892,-0.164;P0.05);C2组术后1月时(r=-0.870;P0.05);C3组术后1月时(r=-0.758;P0.05)。无相关性的有:A组术后1月,3月时(r=0.109,-0.096;P0.05);B1组术后3月时(r=-0.055;p0.05);C1组术后1月,3月时(r=-0.253,-0.364;P0.05);C2组术后3月时(r=-0.544;p0.05);C3组术后3月时(r=-0.469;p0.05)。 结论 1.糖尿病性白内障患者接受超声乳化联合人工晶体植入术后其黄斑中心凹视网膜较单纯老年性白内障患者显著增厚,糖尿病病程10年以内者多在术后3月内恢复。 2.普拉洛芬眼液可减轻白内障超声乳化联合人工晶体植入术后术眼前节炎症反应,对于糖尿病病程10年以上患者还可减轻术后黄斑中心凹网膜增厚程度。 3.在术后黄斑中心凹视网膜厚度异常增厚的患者中,视力与中心凹网膜厚度呈负相关。 4.OCT是研究白内障患者术后黄斑变化的一种理想工具。
