单眼植入AcrySofReSTOR多焦点人工晶状体术后视功能的观察

【摘要】： 背景和目的本文通过研究比较一眼植入AcrySof ReSTOR (SA60D3)多焦点人工晶状体,对侧眼植入AcrySof Natural (SA60AT)单焦点人工晶状体及双眼均植入AcrySof Natural (SA60AT)单焦点人工晶状体术后人工晶体眼的视功能的观察分析,评价单眼植入AcrySof ReSTOR多焦点人工晶状体的临床应用价值。资料和方法观察2008年10月～2009年11月在我院接受白内障超声乳化吸除联合人工晶状体植入的40例(80眼)患者按有否脱镜需求分组,实验组：双眼分别植入AcrySof ReSTOR IOL和AcrySof Natural IOL 20例(40眼),对照组：双眼植入AcrySof Natural IOL 20例(40眼)；均为非同期手术,双眼手术后随访3～5月,观察手术后患者的裸眼远近视力、最佳矫正远近视力、立体视觉、对比敏感度及患者的满意度,以问卷的形式对患者术后的主观感觉进行调查。结果术后1周及术后3月的UCDNV、BCDNV、BCNVA实验组与对照组之间比较差异无统计学意义(χ2=2.975,1.554,1.779,2.256,1.954,1.441；P0.05),实验组在术后1周及术后3月的UCNVA与对照组的差异有统计学意义(χ2=15.667,16.035；P0.01)；术后3月实验组与对照组的同时视功能和融合功能之间比较差异无统计学意义(χ2=0.974,0.974；P0.05)；术后3月UCDNV下的远立体视锐度实验组与对照组比较差异无统计学意义(χ2=2.222；P0.05)；术后3月UCNVA、BCNVA下的近立体视锐度实验组与对照组比较差异无统计学意义(χ2=2.465,2.378；P0.05)；术后1月两组的对比敏感度在各空间频率上差异有统计学意义(P0.05)；术后3月两组的对比敏感度在各空间频率上差异无统计学意义(P0.05)。调查问卷显示,单眼植入AcrySof ReSTOR多焦点人工晶状体术后患者满意度高。结论与双眼均植入单焦点人工晶状体相比,单眼植入AcrySof ReSTOR多焦点人工晶状体组患者在没有损失远视力、远立体视力、没有明显降低对比敏感度、同时视功能和融合功能的前提下,获得了良好的近视力,且近、中距离视功能满意度高,患者脱镜率较高,减少了对眼镜的依赖程度,具有一定的临床应用价值。
[Abstract]:Background and objective to compare the effects of implantation of AcrySof rest (SA60D3) intraocular lens. To evaluate the clinical value of unilateral implantation of AcrySof Natural (SA60AT) single focus intraocular lens and binocular implantation of AcrySof Natural (SA60AT) single focus intraocular lens (IOL). Materials and methods from October 2008 to November 2009, 40 patients (80 eyes) who received phacoemulsification combined with intraocular lens implantation in our hospital were divided into groups according to whether or not the lens was removed. In the experimental group, 20 cases (40 eyes) were implanted with AcrySof natural IOL (40 eyes), and 20 cases (40 eyes) were implanted with AcrySof Natural IOL (40 eyes). The best corrected far and near visual acuity, stereopsis, contrast sensitivity and patient satisfaction were investigated by questionnaire. Results there was no significant difference between the experimental group and the control group at 1 week and 3 months after operation (蠂 ~ 2 / 2.975 ~ 1.554 ~ 1.779 ~ 2.256 ~ 1.9541.441P 0.05). The UCNVA of the experimental group was significantly different from that of the control group at 1 week after operation and 3 months after operation (蠂 ~ 2 ~ (2) ~ (15) 667U ~ (16.035) P ~ (0.01), and there was no significant difference between the experimental group and the control group (蠂 ~ (2) at 1 week and 3 months after operation (蠂 ~ (2) 2.975). There was no significant difference in the simultaneous visual function and fusion function between the experimental group and the control group 3 months after operation (蠂 ~ 2 / 0. 974 ~ 0. 974), but there was no significant difference in the acuity of far stereopsis between the experimental group and the control group 3 months after operation (蠂 2 / 2. 222). There was no significant difference in the near stereopsis acuity between the experimental group and the control group 3 months after operation (蠂 2 / 2.465 / 2.378g P 0.05), but the contrast sensitivity of the two groups was significantly different in all spatial frequencies at 1 month after operation (P0.05). There was no significant difference in contrast sensitivity between the two groups in each spatial frequency 3 months after operation (P0.05). The questionnaire showed that patients with monocular implantation of AcrySof ReSTOR had high satisfaction after multi-focus intraocular lens implantation. Conclusion compared with the single focus intraocular lens implantation in both eyes, the patients with single eyes implanted with AcrySof ReSTOR multifocal intraocular lens have no loss of far vision, distant stereoscopic vision, and no significant decrease in contrast sensitivity. On the premise of both visual function and fusion function, good near vision was obtained, and close and middle distance visual function satisfaction was high, and the rate of devitalization of patients was higher, which reduced the degree of dependence on glasses and had certain clinical application value.
