慢性鼻—鼻窦炎鼻内镜术前术后黏蛋白MUC5AC与MUC5B的表达

【摘要】:目的:检验2种黏蛋白(MUC5AC, MUC5B)在慢性鼻-鼻窦炎患者术前、术后及正常鼻黏膜中的表达情况,探讨黏蛋白对慢性鼻-鼻窦炎鼻腔黏膜超分泌的影响及临床意义。 方法:收集行鼻内镜手术治疗慢性鼻-鼻窦炎患者术前鼻腔上颌窦口黏膜(20例),术后6个月上颌窦口黏膜(20例),正常下鼻甲上缘黏膜(5例)。免疫组织化学(ABC)染色法检测及定位慢性鼻-鼻窦炎患者鼻黏膜中黏蛋白的表达。 结果:通过免疫组织化学染色,观察黏蛋白MUC5AC, MUC5B在鼻黏膜组织中分布情况。结果显示黏蛋白在慢性鼻-鼻窦炎患者鼻黏膜中的表达主要集中在杯状细胞的胞浆(MUC5AC)、黏膜下腺体胞浆(MUC5B)中。术前组的鼻黏膜阳性表达高于术后组,两者差异有统计学意义(p0.05)。术后组及对照组的鼻黏膜阳性表达相接近,两者差异无统计学意义(p0.05)。术前组的鼻黏膜阳性表达高于对照组,两者差异有统计学意义(p0.05) 结论:1.MUC5AC、MUC5B在鼻黏膜中均有表达,其中MUC5AC表达主要集中在杯状细胞的胞浆,MUC5B表达主要集中在黏膜下腺体细胞胞浆。 2.免疫组化结果显示MUC5AC、MUC5B在慢性鼻-鼻窦炎鼻黏膜术前组中表达较术后组和对照组高,MUC5AC、MUC5B在慢性鼻-鼻窦炎黏液中呈高水平分泌。术后6个月时,患者鼻腔黏膜基本完成上皮化,鼻腔未见明显脓性分泌物的情况下,MUC5AC、MUC5B在慢性鼻-鼻窦炎鼻黏膜中的表达下调。
[Abstract]:Objective: to examine the expression of two kinds of mucin (MUC5AC, MUC5B) in the nasal mucosa of patients with chronic rhinosinusitis before and after operation, and to investigate the effect of MUC5AC on the hypersecretion of nasal mucosa in patients with chronic rhinosinusitis and its clinical significance. Methods: 20 patients with chronic rhinosinusitis were treated with endoscopic sinus surgery (20 cases), 20 cases with oral mucosa of maxillary sinus (20 cases) 6 months after operation and 5 cases with normal superior inferior turbinate mucosa (5 cases). The expression of mucin in nasal mucosa of patients with chronic rhinosinusitis was detected by immunohistochemical staining. Results: the distribution of mucin MUC5ACand MUC5B in nasal mucosa was observed by immunohistochemical staining. The results showed that the expression of mucin in nasal mucosa of patients with chronic rhinosinusitis was mainly in the cytoplasm of goblet cells (MUC5AC) and the cytoplasm of submucosal glands (MUC5B). The positive expression of nasal mucosa in preoperative group was higher than that in postoperative group (p 0.05). The positive expression of nasal mucosa in the postoperative group and the control group was similar, and there was no significant difference between the two groups (p 0.05). The positive expression of MUC5ACC5B in the nasal mucosa of the preoperative group was significantly higher than that of the control group (p0.05). Conclusion the positive expression of MUC5ACC5B in the nasal mucosa was significantly higher than that in the control group (p0.05). The expression of MUC5AC was mainly concentrated in the cytoplasm of goblet cells. The results of immunohistochemistry showed that MUC5ACmMUC5B was higher in mucus of chronic rhinosinusitis than that in postoperative group and control group, and the expression of MUC5B in mucus of chronic rhinosinusitis was higher than that of control group. At 6 months after operation, the mucosal epithelization of nasal cavity was basically completed, and the expression of MUC5AC MUC5B was down-regulated in the nasal mucosa of chronic rhinosinusitis without obvious purulent secretion.
