Bcl-2,E-cadherin,N-cadherin蛋白在喉鳞癌中的表达及临床意义

【摘要】:目的:通过检测Bcl-2,E-cadherin,N-cadherin蛋白在喉鳞癌中的表达水平,探讨Bcl-2,E-cadherin,N-cadherin蛋白三者与喉鳞癌发生发展、临床病理特征的关系,为进一步阐明喉鳞癌的发病机制,诊断指标、评估预后提供理论和实验依据。材料与方法:47例人喉鳞癌标本由湖南省肿瘤医院病理科提供,26例声带息肉标本来自南华大学病理教研室,40例正常喉粘膜标本来源南华大学司法鉴定中心。石蜡标本每个切5张白片,分别进行HE染色、免疫组化、阴性对照。采用SP法检测喉鳞癌及非喉癌组织(非典型增生、声带息肉及正常喉粘膜)中Bcl-2,E-cadherin,N-cadherin蛋白的表达,以PBS代替一抗作为阴性对照。 结果: 1、Bcl-2蛋白阳性表达率在喉鳞癌、非典型增生、声带息肉以及正常喉粘膜中分别为61.7%(29/47)、33.3%(11/33)、19.2%(5/26)、0(0/40)。Bcl-2蛋白在喉鳞癌中的表达率远高于在癌旁、息肉及正常喉粘膜的组织。喉鳞癌中Bcl-2蛋白与喉鳞癌的病理组织学分级、临床分期、淋巴结转移、浸润深度等相关(P<0.05),与年龄、性别无关(P0.05)。 2、喉鳞癌、非典型增生、声带息肉及正常喉粘膜中E-cadherin蛋白阳性表达率分别为40.4%(19/47)、66.7%(22/33)、84.7%(22/26)、100%(40/40)。E-cadherin蛋白在喉鳞癌中的表达率远低于在癌旁、息肉及正常的喉粘膜组织。E-cadherin蛋白的阳性表达和喉鳞癌的病理组织学分级、临床分期、淋巴结转移、浸润深度等密切相关(P<0.05),与年龄、性别无关(P0.05)。 3、喉鳞癌、非典型增生、声带息肉及正常喉粘膜中N-cadherin蛋白阳性表达率分别为70.2%(33/47)、42.4%(14/33)、11.5%(3/26)、0(0/40)。N-cadherin蛋白的阳性表达与喉鳞癌的病理学组织分级、临床分期、淋巴结转移、浸润深度等相关(P<0.05),与年龄、性别无关(P0.05)。 4、喉鳞癌中,Bcl-2与E-cadherin蛋白的表达呈负相关(r=-0.421,P=0.003),与N-cadherin蛋白的表达呈正相关(r=0.635, P=0.000), E-cadherin蛋白与N-cadherin蛋白的表达呈负相关(r=-0.411, P=0.004)。 5、生存分析:对有生存资料的47例喉鳞癌患者进行生存分析,我们发现Bcl-2蛋白强阳性表达者生存期明显低于阴性表达者,E-cadherin阴性表达者、N-cadherin蛋白阳性表达者其生存期明显缩短。 结论: 1、喉鳞癌中Bcl-2,N-cadherin蛋白表达上调,E-cadherin蛋白表达下调。 2、Bcl-2,E-cadherin和N-cadherin蛋白的表达与喉鳞癌的临床分期、病理学组织分级、淋巴结癌转移及生存期密切相关。 3、联合检测Bcl-2,E-cadherin,,N-cadherin蛋白在喉鳞癌中表达,对预测喉鳞癌的侵袭转移、判断预后具有重要意义。
[Abstract]:Objective: to investigate the relationship between Bcl-2E-cadherin protein and carcinogenesis and clinicopathological features of laryngeal squamous cell carcinoma (LSCC) by detecting the expression of Bcl-2E-cadherin protein in laryngeal squamous cell carcinoma (LSCC), and to elucidate the pathogenesis of laryngeal squamous cell carcinoma (LSCC). Evaluation of prognosis provides theoretical and experimental evidence. Materials and methods Forty-seven human laryngeal squamous cell carcinoma specimens were collected from the Department of Pathology of Hunan Cancer Hospital. 26 vocal cord polyp specimens were collected from 40 normal laryngeal mucosa specimens from the South China University Forensic Identification Center. Paraffin wax specimens were divided into 5 white slices and stained with HE, immunohistochemistry and negative control respectively. The expression of Bcl-2 E-cadherin N-cadherin protein in laryngeal squamous cell carcinoma (LSCC) and non-laryngeal carcinoma (atypical hyperplasia, vocal cord polyp and normal laryngeal mucosa) was detected by SP method. Results: 1 the positive expression rate of Bcl-2 protein in laryngeal squamous cell carcinoma, atypical hyperplasia, vocal cord polyp and normal laryngeal mucosa was 61.7% (29 / 47) or 33.3% (11 / 33) or 19.2% (5 / 26) / 0 (0 / 40) .Bcl-2 protein expression in laryngeal squamous cell carcinoma was significantly higher than that in adjacent laryngeal squamous cell carcinoma, polyp and normal laryngeal mucosa. Bcl-2 protein in laryngeal squamous cell carcinoma was correlated with histological grade, clinical stage, lymph node metastasis and depth of invasion (P < 0.05), but not with age and sex (P0.05). The positive expression rates of E-cadherin protein in vocal cord polyps and normal laryngeal mucosa were 40.4% (19 / 47), 66.7% (22 / 33), 84.7% (22 / 26) and 100% (40 / 40). Clinical stage, lymph node metastasis, depth of invasion were closely related (P < 0.05), not related to age and sex (P0.05), laryngeal squamous cell carcinoma (LSCC), atypical hyperplasia, and so on. The positive expression rates of N-cadherin protein in vocal cord polyps and normal laryngeal mucosa were 70.2% (33 / 47), 42.4% (14 / 33) and 11.5% (3 / 26), respectively. The positive expression of N-cadherin protein was correlated with the pathological grade, clinical stage, lymph node metastasis, depth of invasion of laryngeal squamous cell carcinoma (P < 0.05). The expression of Bcl-2 and E-cadherin in laryngeal squamous cell carcinoma was negatively correlated with the expression of E-cadherin protein (r-0.421, P0.003), and positively correlated with the expression of N-cadherin protein (r-0.635, P0. 000), and the expression of E-cadherin protein was negatively correlated with the expression of N-cadherin protein (r-0.411, P0. 004). 5. Survival analysis: a positive correlation was found between the expression of E-cadherin protein and the expression of N-cadherin protein (r-0.411, P0. 004). Data of 47 patients with laryngeal squamous cell carcinoma were analyzed. We found that the survival time of patients with strong positive expression of Bcl-2 protein was significantly shorter than that of those with negative expression of E-cadherin protein and positive expression of N-cadherin protein. Conclusion: 1, the expression of Bcl-2mN-cadherin protein up-regulates the expression of E-cadherin protein in laryngeal squamous cell carcinoma, and the expression of E-cadherin protein and N-cadherin protein in laryngeal squamous cell carcinoma is correlated with the clinical stage and pathological grade of laryngeal squamous cell carcinoma. The expression of Bcl-2E-cadherin N-cadherin protein in laryngeal squamous cell carcinoma (LSCC) was closely related to its metastasis and survival time. 3. The expression of Bcl-2E-cadherinN-cadherin protein in laryngeal squamous cell carcinoma was significant in predicting invasion and metastasis of laryngeal squamous cell carcinoma and judging prognosis.
