特发性黄斑裂孔内界膜手术对视觉功能影响的研究

【摘要】:目的:检测特发性黄斑裂孔手术方法中,黄斑内界膜剥除与否对术眼视觉功能的影响。探讨特发性黄斑裂孔手术治疗的安全有效方法及其适应症。 方法:明视负波(PhNR)是一项检测神经节细胞和视神经纤维损害的功能变化的有效手段。对连续收治的病程在6个月以内的Ⅲ、Ⅳ期特发性黄斑裂孔,行经睫状体平坦部三通道玻璃体切除联合无染色剂辅助的黄斑区视网膜内界膜剥除术,术后眼内气体填充、面向下体位。除常规眼科检查外,术前及术后3个月检测记录蓝色背景红色刺激光条件下视网膜电流图(ERG),比较手术前、手术后视网膜电流图中α、β波和明视负波(PhNR)的振幅变化。对同期收治的Ⅱ期特发性黄斑裂孔行单纯玻璃体切除术,切除玻璃体后皮质,术后眼内气体填充、面向下体位。相同方法检测、比较手术前、手术后视网膜电流图α、β波和明视负波(PhNR)的振幅变化。随访6~12个月。观察黄斑内界膜剥除手术前后视觉功能的影响,黄斑内界膜未剥除手术前后视觉功能的变化。统计学方法:所有数据均采用SPSS13.0软件进行数据分析。手术前后LogMAR视力对比采用Wilcoxon配对秩和检验;手术前后ERG各波振幅对比采用Wilcoxon Signed Ranks Test。以P0.05为有统计学意义。结果:对连续收治的Ⅲ、Ⅳ期特发性黄斑裂孔32例(32眼),术前平均裂孔直径375.6μm(352μm~445μm),手术后裂孔解剖闭合30眼(30/32,93.6%),平均最佳矫正视力(BCVA)提高0.24(术前0.10±0.05,术后0.34±0.17),手术后PhNR平均振幅值较手术前下降12.1μν(术前33.6±3.9μν,术后21.5±6.5μν,P0.05),α、β波振幅略有升高,但无统计学意义(P0.05)。其中40%病人术后短期内闭眼时眼前出现图像闪烁感,术后注视小视标时自觉闪烁感。Ⅱ期特发性黄斑裂孔26例(26眼),术前平均裂孔直径258.4μm(230μm~292μm),手术后裂孔解剖闭合25眼(25/26,96.2%),平均BCVA提高0.31(术前0.33±0.10,术后0.64±0.19),手术后PhNR平均振幅值较手术前下降1.0μν(术前33.9±2.0μν,术后32.9±2.1μν,P0.05),α、β波振幅略有升高,但无统计学意义(P0.05)。结论:特发性黄斑裂孔视网膜内界膜剥除术使Ⅲ、Ⅳ期特发性黄斑裂孔手术后裂孔闭合、视力明显提高,手术后视觉电生理存在一定的视网膜内层功能的损害,但与其在裂孔闭合后视力明显提高的意义相比影响甚小。而Ⅱ期特发性黄斑裂孔无需进行内界膜剥除术,即可获得良好的裂孔闭合率及视觉功能质量。
