P物质在真菌性角膜溃疡中的表达及意义

【摘要】: 目的检测大鼠真菌性角膜溃疡和细菌性角膜溃疡中SP的表达变化过程及差异,探讨SP在真菌性角膜溃疡发病过程中的作用。 方法55只Wistar大鼠随机分为空白对照组(A),实验一组(B),实验二组(C),右眼为实验眼,左眼为对照眼。A组5只大鼠不做任何处理;B组为真菌性角膜溃疡组25只,接种镰刀菌;C组为细菌性角膜溃疡组25只,接种金黄色葡萄球菌;对照眼以生理盐水代替菌株。每日裂隙灯观察眼表情况,并刮取B组、C组溃疡边缘及底部组织涂片行革兰染色和10%氢氧化钾涂片,同时将刮取物接种于沙保弱培养基上25℃培养以确定真菌或细菌感染。详细记录各组大鼠角膜溃疡生长情况。分别于造模后1d、5d、8d、10d、14d随机抽取各组大鼠取材。HE染色观察角膜溃疡组织学变化。免疫组织化学方法和RT-PCR法检测SP的表达。 结果1、B组1d形成灰白色溃疡灶,可见免疫环;5d为溃疡高峰期,可出现穿孔;8d时溃疡缩小;10d有新生血管生长,14d形成血管化角膜白斑。C组变化与B组时间变化类似,但C组充血更严重,无免疫环。对照眼1d角膜水肿,5d角膜透明,未见感染。 2、HE染色示B组1d溃疡形成,基质浅层胶原纤维排列紊乱,中性粒细胞浸润。5d溃疡加重,整个基质层大量炎性细胞浸润;8d溃疡缩小,基质炎症细胞减少,成纤维细胞增生,见新生血管形成;10d新生血管成熟;14d基质层纤维呈瘢痕样重构。C组变化类似,炎细胞浸润较多。 3、免疫组化示:(1)正常角膜上皮细胞中中等量表达SP,基质层及内皮有微弱表达(2)B组中角膜内皮细胞、炎症细胞、新形成的血管内皮细胞,成纤维母细胞中SP表达均增强,8d最强,10d下降,14d降至低于正常,差异有统计学意义(P0.05)(3)C组其变化趋势与B组中类似,但相同时间点两组间比较,B组低于C组,差异有统计学意义(P0.05)(4)对照眼1d达到最高,5d降至正常水平。各时间点B组,C组实验眼与对照眼之间比较差异有统计学意义(P0.05)。 4、RT-PCR与免疫组织化学检测的变化趋势一致。结论真菌性角膜溃疡中SP表达早期与溃疡进展一致,晚期与溃疡修复一致,提示SP可能参与了真菌性角膜溃疡早期损伤和晚期修复过程;其表达低于同时间点的细菌性角膜溃疡,提示其表达水平较低可能参与了真菌性角膜溃疡疼痛较轻的机制。
