非球面晶体与球面晶体植入术后视觉质量的对比研究

【摘要】: 目的对比研究白内障超声乳化联合人工晶状体植入手术后非球面晶状体与球面晶状体眼的视觉质量。 方法将我院2009年4月至2010年4月拟行超声乳化白内障吸除联合人工晶状体植入术的年龄相关性白内障患者106例随机分为2大组:非球面晶状体组与球面晶状体组。其中非球面晶状体组随机分别植入球差值-0.18μm非球面人工晶状体(HOYA60AD),球差值-0.15~-0.18μm非球面人工晶状体(STAAR KS-3AI),零球差非球面人工晶状体(Rayner920H或Rayner970C),非恒定球差非球面人工晶状体(Zeiss ZO);传统球面人工晶状体组随机分别植入Rayner570C,XLSTABI-SKY,Hexavision HQ-201人工晶状体。术后1个月对患者进行裂隙灯下眼前段和眼底检查、电脑验光及最佳矫正视力检查,对除外眼部并发症及裸眼视力或最佳矫正视力≥0.8患者行明视和暗视眩光及无眩光条件下的对比敏感度、6mm瞳孔直径时眼的总体像差检测和视功能与生存质量问卷调查。 结果术后1个月,7组人工晶状体患者最佳矫正远视力差异无统计学意义(P0.05);明视、明视眩光中低空间频率和暗视高空间频率下4种非球面晶状体组对比敏感度值总体高于球面晶状体组。四种非球面晶体间比较:在明视及暗视有眩光或无眩光的各空间频率下非恒定球差和球差值-0.15~-0.18μm非球面人工晶状体组相较球差值-0.18μm非球面人工晶状体组及零球差非球面人工晶状体组对比敏感度值高,其中非恒定球差非球面人工晶状体组在明暗视中、低空间频率高,球差值-0.15~-0.18μm非球面人工晶状体组在明视中高空间频率及暗视低空间频率高。三组球面晶状体组比较:明视各空间频率和暗视眩光低空间频率时XLSTABI-SKY对比敏感度值高,暗视眩光中高空间频率下Rayner570C相对较高,但与XLSTABI-SKY差异无统计学意义。Zeiss非球面晶体在明视和暗视有无眩光时中高空间频率下相较Zeiss球面晶体对比敏感度值高。而Rayner非球面晶体在明视有无眩光的中低空间频率下对比敏感度值高于Rayner球面晶体,但在明视高空间频率及暗视各空间频率下Rayner球面晶体对比敏感度值高于Rayner非球面晶体。术后1个月,瞳孔直径6mm情况下非球面人工晶状体组的总眼球差为(0.17士0.25)μm,球面人工晶状体组为(0.31士0.24)μm,差异有统计学意义(P0.05)。VF/QOL问卷调查显示7组人工晶状体眼之间的视功能与生存质量差异有统计学意义(P0.05 )。 结论超声乳化白内障吸除联合人工晶状体植入术后,非球面人工晶状体相较球面晶状体可以降低总眼球差,相对提高视觉对比度;其中非恒定球差非球面人工晶状体和球差值-0.15~-0.18μm非球面人工晶状体与本研究中其他非球面晶状体相比,能够相对明显的提高白内障患者术后明暗对比敏感度,改善患者视觉质量。
[Abstract]:Objective to compare the visual quality of aspherical lens and spherical lens after phacoemulsification combined with intraocular lens implantation. Methods from April 2009 to April 2010, 106 patients with age-related cataract undergoing phacoemulsification and intraocular lens implantation were randomly divided into two groups: aspheric lens group and spherical lens group. The aspheric lens group was randomly implanted with spherical difference (-0.18 渭 m), spherical difference (-0.15 渭 m) -0.18 渭 m aspheric intraocular lens (STAAR KS-3AI), zero spherical aberration aspheric intraocular lens (Rayner920H or Rayner970C) and unsteady spherical aberration aspheric intraocular lens (Rayner920H or Rayner970C). The conventional spherical intraocular lens (Zeiss ZO);) group was randomly implanted with Rayner 570 Con XLSTABI-SKYY Hexavision HQ-201 intraocular lens. One month after operation, the patients were examined with slit lamp under anterior segment and fundus, computer optometry and best corrected visual acuity (BCVA). The total aberration, visual function and quality of life (QOL) of eyes with 6 mm pupil diameter were measured with contrast sensitivity and dark glare and without glare in patients with ocular complications and uncorrected visual acuity or best corrected visual acuity (BCVA) 鈮,
