PTEN、MMP-9、Survivin和Ki67在视网膜母细胞瘤中的表达和意义

【摘要】: 目的探讨PTEN、MMP-9、Survivin和Ki67在视网膜母细胞瘤中的表达及其与临床病理因素的关系 方法采用免疫组织化学方法检测41例视网膜母细胞瘤切片和10例正常视网膜组织中PTEN、MMP-9、Survivin和Ki67的表达情况,分析四者分别表达及共表达与视网膜母细胞瘤患者性别、临床分期、视神经受侵犯情况和分化程度的关系。所有数据应用SPSS13.0软件进行统计。 结果 1.免疫组织化学染色结果表明:视网膜母细胞瘤中PTEN、MMP-9、Survivin和Ki67的阳性表达率分别为68.3%,60.9%,63.4%和43.9%, PTEN表达明显降低,MMP-9、Survivin和Ki67表达明显增高,与对照组比较差异具有统计学意义(P0.01)。经Pearsonχ2检验显示,不同性别之间PTEN、MMP-9、survivin和Ki-67表达均无统计学差异(p0.05)。 2.采用Fisher's Exact Test检验显示,眼内生长期PTEN表达阳性率(90.0%)和眼内压增高期阳性率(84.2%)相比,差异无统计学意义(P0.05);眼内生长期与眼外扩展期PTEN表达阳性率比较,差异有显著统计学意义(P0.05);眼内压增高期与眼外扩展期比较,差异有统计学意义(P 0.05)。PTEN在伴有视神经浸润的RB组织中的表达阳性率明显低于不伴视神经浸润者,差异有统计学意义(P0.05)。在RB组织分级之间,分化型RB组织中的PTEN蛋白表达阳性率明显高于未分化型,差异有统计学意义(P0、05)。眼内生长期,MMP-9表达阳性率(40.0%)和眼内压增高期阳性率(52.6%)相比,差异无统计学意义(P0.05);眼内生长期与眼外扩展期比较,差异有显著统计学意义(P0.05);眼内压增高期与眼外扩展期比较,差异有统计学意义(P0.05)。MMP-9在伴有视神经浸润的RB组织中,表达阳性率明显高于不伴视神经浸润者,差异有统计学意义(P0.05)。在RB组织分级之间,分化型RB组织中MMP-9表达阳性率明显低于未分化型,差异有统计学意义(P0.05)。survivin的表达阳性率与患者的性别、临床分期、组织学分型差异无统计学意义(P0.05)。眼内生长期和眼内压增高期Ki67阳性表达率明显低于眼外扩展期,差异具有统计学意义(P0、05)。Ki67在伴有视神经浸润的RB组织中,表达阳性率明显高于不伴视神经浸润者,差异有统计学意义(P0.05)。在RB组织分级之间,分化型RB组织中Ki67表达阳性率明显低于未分化型,差异有统计学意义(P0.05)。 经spearman等级相关分析,性别与MMP-9、survivin、Ki-67共表达的相关性不存在统计学差异(rs=-0.13,p0.05)。临床分期与MMP-9、survivin、Ki-67共表达存在正相关(rs=0.434,p<0.05)。视神受经侵犯程度与MMP-9、survivin、Ki-67共表达存在正相关(rs=0.470,p0.05。组织分化程度与MMP-9、survivin、Ki-67共表达存在负相关(rs=-0.566,p0.01)。PTEN表达与MMP-9、survivin、Ki-67共表达存在负相关(rs=-0.542,p0.01)。结论PTEN在视网膜母细胞瘤组织中的阳性表达率显著降低,MMP-9、Survivin和Ki-67在视网膜母细胞瘤组织中的阳性表达率显著高于正常视网膜组织,四者单独表达或共表达与视网膜母细胞瘤的发生、侵袭和转移有关。
[Abstract]:Objective to investigate the expression of survivin and Ki67 in retinoblastoma and their relationship with clinicopathological factors. Expression of survivin and Ki67 in membrane tissue To analyze the relationship between the expression and coexpression of retinoblastoma and sex, clinical stage, optic nerve invasion and degree of differentiation. All data are counted by SPSS13.0 software. Result 1. The results of immunohistochemical staining showed that the positive rates of survivin and Ki67 in retinoblastoma were 68.3% and 43.9%, respectively. The expression of PTEN significantly decreased the expression of MMP-9, survivin and Ki67 in retinoblastoma, which was significantly higher than that in control group (P0.01). Pearson 蠂 2 test showed that there was no significant difference in the expression of MMP-9, survivin and Ki-67 between different genders (p0.05). The positive rate of PTEN expression in intraocular growth stage (90.0%) was not significantly higher than that in intraocular hypertension stage (84.2%) (P0.05), and the positive rate of PTEN expression in intraocular growth phase was higher than that in extraocular expansion stage (P 0.05). The difference was statistically significant (P0.05), the positive rate of PTEN expression in RB with optic nerve infiltration was significantly lower than that in patients without optic nerve infiltration. The difference was statistically significant (P0.05). The positive rate of PTEN protein expression in differentiated RB tissues was significantly higher than that in undifferentiated RB tissues (P0. 05). The positive rate of MMP-9 expression in intraocular growth stage (40.0%) was not significantly different from that in intraocular pressure (52.6%) period (P0.05), and there was significant difference between intraocular growth phase and extraocular dilatation stage (P0.05). The positive rate of MMP-9 in RB with optic nerve invasion was significantly higher than that without optic nerve infiltration (P0.05). The positive rate of MMP-9 expression in differentiated RB tissues was significantly lower than that in undifferentiated RB tissues (P0.05). There was no significant difference between the positive rate of MMP-9 expression and gender, clinical stage and histological classification (P0.05). The positive expression rate of Ki67 in the stage of intraocular growth and increased intraocular pressure was significantly lower than that in the stage of extraocular expansion (P005) .Ki67 was significantly higher in RB tissues with optic nerve infiltration than in those without optic nerve infiltration. The difference was statistically significant (P0.05). The positive rate of Ki67 expression in differentiated RB was significantly lower than that in undifferentiated RB (P0.05). By spearman grade correlation analysis, there was no statistical difference between sex and MMP-9 survivin Ki-67 co-expression (rs-0.13p0.05). Conclusion the positive expression rate of PTEN in retinoblastoma is significantly lower than that in normal retinoma. Expression alone or coexpression in four patients was related to the occurrence, invasion and metastasis of retinoblastoma.
