先天性白内障术后后囊混浊的相关因素分析

【摘要】:目的:分析先天性白内障术后导致晶状体后囊混浊(posterior capsular opacities,PCO)的相关因素。方法:采用回顾性的分析方法,对52例(87眼)先天性白内障患儿进行分析,年龄3月~12岁不等,平均年龄4.7岁。其中行白内障超声乳化(Phaco)+后囊连续环形撕囊(PCCC)22例(30眼);行白内障超声乳化(Phaco)+后囊连续环形撕囊(PCCC)+前部玻璃体切除术(AV)组30例(57眼)。术后Ⅰ期植入人工晶体(IOL)19例(27眼);无晶体眼15例(27眼);Ⅱ期植入IOL18例(33眼)。手术年龄小于等于2岁的22例(30眼);年龄大于2岁的例30例(57眼)。术后随访6个月,观察患者晶状体后囊膜的变化。结果:至术后6个月末,18眼(20.69%)发生PCO,其中Phaco+PCCC组11眼(36.67%), Phaco+PCCC+AV组7眼(12.28%),两组结果分析,x2=7.123,P0.05,差异有统计学意义。Ⅰ期IOL植入5眼(18.52%),无晶体眼5眼(18.52%),Ⅱ期IOL植入8眼(24.24%),经x2检验,x2=0.409,P0.05,差异无统计学意义。手术年龄小于等于2岁13眼(36.11%),2岁以上5眼(9.80%),经z-检验,x2=8.901,P0.05,差异有统计意义。结论:先天性白内障术后PCO的发生与手术方式及手术年龄有关。前部玻璃体切除技术的应用可以明显减低术后PCO的发生率;手术年龄越小,术后PCO的发生率越高。

Methods: 52 children (87 eyes) with congenital cataract were analyzed retrospectively. The age ranged from 12 years to 12 years, with an average age of 4.7 years. There were 22 cases (30 eyes) of continuous circular capsulorhexis (PCCC) with cataract phacoemulsification (Phaco) and 30 cases (57 eyes) of (AV) group treated with phacoemulsification (Phaco) posterior capsular continuous circular capsulorhexis (PCCC) anterior vitrectomy. Intraocular lens (IOL) was implanted in 19 cases (27 eyes), aphakic eyes in 15 cases (27 eyes) and IOL18 implantation in 33 eyes (33 eyes). The operative age was less than 2 years in 22 cases (30 eyes) and over 2 years in 30 cases (57 eyes). Patients were followed up for 6 months to observe the changes of posterior capsule of lens. Results: PCOs occurred in 18 eyes (20.69%) at the end of 6 months after operation, including 11 eyes (36.67%) in Phaco PCCC group (7 eyes (12.28%) in), Phaco PCCC AV group).
