TNF-α、MMP-1在中耳胆脂瘤中的表达及意义

【摘要】:目的 检测肿瘤坏死因子-α(TNF-α)和基质金属蛋白酶-1(MMP-1)在中耳胆脂瘤上皮中的表达情况,探讨二者在胆脂瘤上皮增殖和骨质破坏中的作用及其相关性,从分子水平揭示胆脂瘤的发病机制。 方法 本研究通过收集35例伴有胆脂瘤的中耳乳突炎患者手术中切除并经常规病理确诊的中耳胆脂瘤标本,迅速固定备用;手术中观察骨质破坏的程度,按照骨质的破坏程度分为四组:Ⅰ度:6例,Ⅱ度:11例,Ⅲ度:14例,Ⅳ度:4例。同期,取15例乳突根治术患者的正常外耳道皮肤作为对照组。采用免疫组织化学技术(SP法)检测TNF-α及MMP-1在35例中耳胆脂瘤上皮组织和15例正常外耳道皮肤中的表达情况,并应用计算机图像分析系统对TNF-α和MMP-1染色结果进行定量分析;同时进一步分析中耳胆脂瘤组织中TNF-α和MMP-1二者之间的关系。 结果 1. TNF-α蛋白在胆脂瘤组织的表达明显高于正常外耳道皮肤,TNF-α在中耳胆脂瘤组织中的表达量为132.80±18.90,在正常外耳道皮肤中的表达量仅为74.59±13.72,二者之间的差异具有显著性(t=10.928,P0.05)。 2.MMP-1阳性染色为胞膜和胞浆出现棕黄色颗粒状,MMP-1蛋白在胆脂瘤组织的表达明显高于正常外耳道皮肤,MMP-1在中耳胆脂瘤组织中的表达水平为126.76±12.64,而其在正常外耳道皮肤中的表达水平为92.56±12.13,两者之间差异有统计学意义(t=7.382,P0.05)。 3. TNF-α、MMP-1蛋白在中耳胆脂瘤组织中随着骨质的破坏程度增高,其表达量也随之增高,各组之间的平均光密度值进行两两比较,差异具有统计学意义(P0.05)。 4. TNF-α与MMP-1蛋白在中耳胆脂瘤组织中的阳性表达通过相关分析发现二者之间呈正相关(r=0.323,P0.05)。 结论 1. TNF-α及MMP-1蛋白在中耳胆脂瘤上皮中的高表达,提示二者可能在胆脂瘤上皮的增殖及骨质破坏的过程中发挥重要作用。 2. TNF-α及MMP-1蛋白在中耳胆脂瘤组织中随着周围骨质破坏程度的范围扩大,二者表达量也随之增高,表明二者在胆脂瘤上皮对骨质的侵蚀行为中起非常重要的作用。 3. TNF-α与MMP-1之间在中耳胆脂瘤组织中的阳性表达呈正向相关,提示二者在胆脂瘤组织中的增殖和侵蚀行为中可能有正向协同作用。
