那他霉素联合氟康唑及碘酊烧灼治疗真菌性角膜溃疡的临床研究

【摘要】:目的:研究那他霉素滴眼液对真菌性角膜溃疡的临床治疗效果,探讨真菌性角膜溃疡的现代诊断与治疗手段。 方法:对60例(60眼)已确诊为真菌性角膜溃疡的23~69岁的患者随机分为治疗组30例(30眼)与对照组30例(30眼)。对照组应用氟康唑及碘酊烧灼法,治疗组同时给予那他霉素滴眼液,对两组患者治疗2个月后的疗程和疗效进行比较。采用德国海德堡共焦激光显微镜对比分析真菌菌丝的数量变化。对于数据结果进行统计学分析,应用SPSS软件进行统计学处理,结果以P0.05为差异有统计学意义。 结果:(1)疗程方面:治疗组2个月内治愈21例,疗程29~55d,平均44.02±7.59d。对照组2个月内治愈12例,疗程43~60d,平均51.95±8.35d。两组对比差异具有统计学意义(P0.05),治疗组优于对照组。(2)疗效方面:治疗组轻型8眼,治愈8眼;中型15眼,治愈11眼,显效2眼,有效2眼;重型7眼,治愈2眼,显效2眼,有效2眼,无效1眼。对照组轻型8眼,治愈8眼;中型14眼,治愈4眼,显效4眼,有效4眼,无效2眼;重型8眼,有效3眼,无效5眼。两组有效率对比差异具有统计学意义(P0.05),治疗组优于对照组。 结论:那他霉素联合氟康唑及碘酊烧灼是治疗真菌性角膜溃疡的有效方法,有助于缩短疗程、提高治疗的有效率,为治疗真菌性角膜溃疡提供了一种理想的治疗方案。
[Abstract]:Objective:To study the clinical effect of Natamycin eye drops on fungal corneal ulcer and to explore the modern diagnosis and treatment of fungal corneal ulcer.
Methods: 60 patients (60 eyes) with fungal corneal ulcer were randomly divided into treatment group (30 eyes) and control group (30 eyes). The control group was treated with fluconazole and iodine tincture cauterization, and the treatment group was treated with natamycin eye drops at the same time. Deborough confocal laser microscopy was used to analyze the changes of mycelium number. Statistical analysis was made on the data and SPSS software was used to analyze the results. The difference was statistically significant with P 0.05.
Results: (1) In the treatment group, 21 cases were cured within 2 months, the treatment course was 29-55 days, the average was 44.02+7.59 days. Effective 2 eyes, effective 2 eyes; severe 7 eyes, cured 2 eyes, markedly effective 2 eyes, effective 2 eyes, ineffective 1 eye. Control group light 8 eyes, cured 8 eyes; medium 14 eyes, cured 4 eyes, markedly effective 4 eyes, effective 4 eyes, ineffective 2 eyes; severe 8 eyes, effective 3 eyes, ineffective 5 eyes.
Conclusion: Natamycin combined with fluconazole and iodine tincture is an effective method to treat fungal corneal ulcer. It is helpful to shorten the course of treatment and improve the efficiency of treatment.
