人工耳蜗术后构音能力评估

【摘要】:目的:1.评估构音器官运动功能;2.评价构音语音能力;3.分析构音障碍的主要问题所在,为之后的言语矫治提供依据;4.测试平均言语基频及响度; 方法:1.客观测量口腔轮替运动速率、下颌距和舌距,并与听力正常组进行对比;2.用黄昭鸣-韩知娟词表进行测试,通过分析得出言语清晰度,并填写音位对比表,最小音位对比习得表,声母音位习得表,并与正常对照;3.用实时言语测量仪测试言语平均基频和响度。 结果: 1.耳蜗使用时间48月组的构音器官的运动功能,低于听力正常组,差异具有统计学意义(p 0.05),耳蜗使用时间60月组和72月组仍低于听力正常组,但是没有显著性差异;2.构音清晰度和术后人工耳蜗使用时间、术后康复时间在0.01水平上显著相关;3.在耳蜗使用时间37~72月组,植入年龄越小,清晰度越好,在1~36月组,植入年龄3~6岁者清晰度较好;4.声母中,从发音方式看,错误率最高的是塞擦音,其次是擦音;从发音部位看错误率最高的是舌尖音和舌根音;5.55个被试者中53人的言语强度在65dB以下;6.耳蜗使用组48月组和听力正常组具有显著性差异(p0.05),60月组和72月组差异性不大(p0.05); 结论:1.人工耳蜗使用者构音器官的运动功能需5~6年接近听力正常者; 2.术后康复时间时间越长,构音清晰越好; 3.塞擦音和擦音的错误率高;舌尖音和舌根音的错误率高; 4.受试者的响度异常;5.耳蜗使用时间48月组的平均言语基频低于听力正常组。
[Abstract]:Objective: 1. To evaluate the motor function of sound organs. 2. To evaluate the phonetic ability of structuring sound; 3. Analyze the main problems of dysarthria and provide the basis for speech correction. 4. The average speech frequency and loudness were measured. Oral rotation rate, mandibular distance and tongue distance were measured objectively and compared with normal hearing group. 2. By using Huang Zhaoming-Han Zhijuan word table, the speech articulation is obtained, and the phoneme contrast table, the minimum phoneme contrast acquisition table, the consonant phoneme acquisition table and the normal control are filled out. 3. The average fundamental frequency and loudness of speech were measured by real-time speech measuring instrument. Results: 1. The motor function of cochlea in 48 months group was lower than that in normal hearing group (p 0.05). The cochlear usage time of 60 months group and 72 month group was still lower than that of hearing normal group, but there was no significant difference. 2. Articulation and postoperative cochlear implantation time, postoperative rehabilitation time at 0.01 level significantly correlated; 3. In the 37 ~ 72 months cochlear implantation group, the younger the implantation age, the better the articulation, and in the 1-36 months group, the cleavage was better in the 36-year-old group. 4. Among the consonants, the highest error rate was the syllable, followed by the erasure; the highest error rate was the tip of the tongue and the root of the tongue, the intensity of the speech of 53 of the 5.55 subjects was below 65dB; 6. The cochlear use group had significant difference between 48 months group and normal hearing group (p0.05), but there was no significant difference between 60-month group and 72-month group (p0.05). Conclusion: 1. The motor function of the auditory organ of the cochlear implant needs 5 ~ 6 years to be close to the normal hearing; 2. The longer the postoperative recovery time, the better the articulation. 3. Erasure and erasure error rate is high; tongue tip sound and tongue root sound error rate is high; 4. The loudness of the subjects was abnormal; The average speech frequency of 48-month cochlear use group was lower than that of normal hearing group.
