真菌性角膜炎角膜新生血管形成与预后的研究

【摘要】:目的通过观察兔眼角膜真菌感染后角膜新生血管形成规律,探讨角膜新生血管形成与角膜炎预后的关系。 方法选取7只健康新西兰大白兔,制作标准真菌性角膜炎动物模型。每天在裂隙灯显微镜下观察兔眼大体改变,分别记录真菌溃疡形成后不同时间段兔眼的病程进展;分别于真菌溃疡形成后第3、7、15和30天,行病理学和超微结构检查。 结果兔眼真菌溃疡形成后第1天和第3天,可见有较重的炎症表现;第7天时,均有大量角膜周边新生血管长入;第15天时,球结膜充血减轻,角膜新生血管有所缓解,角膜溃疡未见减轻;第30天时,有较轻的眼部炎症,角膜周边残存少量新生血管。病理学检查显示:真菌溃疡形成后第3天,重度炎症病理表现,毛细血管扩张,无新生血管;第7天,有角膜新生血管形成;第15天炎症局限,有成熟的新生血管;第30天,新生血管减少。超微结构观察:真菌溃疡形成后第3天和第7天,典型炎症的超微结构改变;第15天,基质层仍可见到真菌菌丝,但较前明显减少,角膜上皮细胞核和细胞器异常;第30天,轻度炎症表现,未见真菌菌丝。 结论1兔眼真菌性角膜炎在不应用任何抗真菌药物的情况下具有自愈转归的特点。2兔眼角膜新生血管与真菌性角膜炎的恢复有相关性。
[Abstract]:Objective to investigate the relationship between corneal neovascularization and keratitis prognosis by observing corneal neovascularization after fungal infection. Methods 7 healthy New Zealand rabbits were selected to make standard fungal keratitis animal model. The gross changes of rabbit eyes were observed under slit-lamp microscope every day, and the progress of the disease course of rabbit eyes was recorded at different times after the formation of fungal ulcers, and the pathological and ultrastructural examinations were performed on the 15th and 30th days after the formation of fungal ulcers. Results on the 1st and 3rd day after the formation of fungal ulcer in rabbit eyes, severe inflammation was observed, and on the 7th day, a large number of neovascularization appeared around the cornea. On the 15th day, the bulbar conjunctiva hyperemia was alleviated, corneal neovascularization was alleviated, corneal ulcer was not alleviated, on the 30th day, there was mild ocular inflammation and a small amount of neovascularization around the cornea. Pathological examination showed that on the third day after the formation of fungal ulcer, severe inflammatory pathological manifestations were found, capillary dilated without neovascularization, corneal neovascularization on day 7, inflammation localized on day 15, and mature neovascularization on day 15. On day 30, neovascularization was reduced. Ultrastructural observation: on the 3rd and 7th day after the formation of fungal ulcer, the ultrastructural changes of typical inflammation were observed, and on the 15th day, mycelium could still be seen in the stroma layer, but the number of mycelium was significantly decreased, and the keratoepithelium nucleus and organelle were abnormal. On the 30th day, mild inflammation and no fungal hyphae were found. Conclusion (1) mycological keratitis has the characteristics of self-healing without any antifungal drugs. 2 the corneal neovascularization is correlated with the recovery of fungal keratitis.
