α-2b干扰素与5-氟尿嘧啶在青光眼滤过手术中抗瘢痕作用的对比研究

【摘要】：目的评价α-2b干扰素( Interferonα-2b , IFNα-2b )与5-氟尿嘧啶(5-Fluorouracil ,5-Fu)用于青光眼滤过术的抗瘢痕效果。方法采用随机对照法,将临床收治的52例(56只眼)初次手术的青光眼患者分为IFNα-2b组(28眼)与5-Fu组(28眼)。4例为同一患者左右眼对照。均行小梁切除术。前组用含50万U /mlIFNα-2b,后组用浓度12.5 mg/ml 5-Fu,均术中一次性使用,共4 min。术后根据情况结膜下注药并针刺分离,记录患者眼压、视力、并发症等情况。随访时间4个月。结果两组眼压术后与术前相比均显著降低(P㩳0.001);5-Fu组术后视力下降者11只眼(39.3%),而IFNα-2b组3只眼(10.7%)。两组间差异有统计学意义(2=6.095,P㩳0.05);IFNα-2b组形成的滤过泡以II型为主,5-Fu组则以I型为主(χ2=8.096,P㩳0.05 );IFNα-2b组角膜上皮损伤程度明显轻于5-Fu组(P㩳0.05)。结论IFNα-2b与5-Fu应用于青光眼滤过手术中均能阻止术后滤过通道瘢痕化,降低手术失败率,且IFNα-2b副作用更小,是抗青光眼手术的理想用药。
[Abstract]:Objective to evaluate the anti-scar effect of Interferon 伪 -2b (IFN 伪 -2b) and 5-Fluorouracil (5-Fu) in glaucoma filtration surgery. Methods Fifty-two patients (56 eyes) with primary glaucoma were randomly divided into IFN 伪 -2b group (28 eyes) and 5-Fu group (28 eyes). All patients underwent trabeculectomy. The former group was given 500000 U / mlIFN 伪 -2b, and the latter group was given 12.5 mg/ml 5-Fu.All of them were used once during operation for a total of 4 min.. The intraocular pressure, visual acuity and complications were recorded according to the condition of conjunctival injection and acupuncture separation. The follow-up time was 4 months. Results the postoperative IOP in both groups was significantly lower than that before operation (P0. 001), the visual acuity of 11 eyes (39. 3%) in 5-Fu group and 3 eyes (10. 7%) in IFN 伪-2b group. There was a significant difference between the two groups (2: 6.095P0. 05); IFN 伪 -2b group). The main type of bleb was II type, while that of 5-Fu group was I type (蠂 2 + 8.096%). The degree of corneal epithelial injury in IFN 伪-2b group was significantly less than that in 5-Fu group (P0. 05). Conclusion both IFN 伪 -2b and 5-Fu can prevent the scarring of postoperative filtering channels and reduce the failure rate of operation, and IFN 伪 -2b is an ideal drug for glaucoma surgery.
