改良双腔管在小切口白内障手术中的应用

【摘要】：目的:探讨一款经笔者改良后兼具灌注、娩核、抽吸等多种功能的双腔管在小切口白内障手术中的应用价值。方法:应用改良双腔管,对188例188眼进行白内障摘除手术,观察灌注下撕囊、一次性娩核、水灌注下人工晶体植入等操作的成功率,以及手术后效果和相关并发症的发生情况。结果:应用改良双腔管进行小切口白内障手术,一次性娩核成功率为89%;水灌注下环形撕囊成功率为93.6%;水灌注下人工晶体植入成功率为95.7%,手术并发症主要为后囊破裂(2.7%)、角膜水肿(14.9%)、人工晶体前膜形成(0.1%)。术后第1、7天视力在0.3以上者分别为141眼(75%)和178眼(94.9%);术后3个月角膜散光度为(0.71±0.59)D。结论:自行改良的双腔管具有多功能、操作简便、易于掌握等优点,手术成功率高,术后效果好,是小切口白内障手术中理想的手术器械。
[Abstract]:Objective: to investigate the application value of a modified dual cavity tube with multiple functions of perfusion, nucleus delivery and aspiration in small incision cataract surgery. Methods: 188 eyes of 188 cases with cataract were treated with modified double lumen tube. The successful rate of operation such as capsulorhexis, one-off nucleus delivery and intraocular lens implantation under water perfusion were observed, as well as the results of the operation and the occurrence of related complications. Results: the success rate of one-off nucleus delivery was 89 and the success rate of circular capsulorhexis was 93.6 under water perfusion. The successful rate of intraocular lens implantation was 95.7%. The main complications were posterior capsule rupture (2.7%), corneal edema (14.9%) and intraocular lens anterior membrane formation (0.1%). The visual acuity was better than 0.3 in 141 eyes (75%) and 178 eyes (94.9%) on the 1st day after operation, and the corneal astigmatism was (0.71 卤0.59) D at 3 months after operation. Conclusion: the self-modified double-lumen tube has the advantages of multifunction, simple operation, easy to master, high success rate and good postoperative effect. It is an ideal instrument for small incision cataract surgery.
