SBK与PRK矫正中低度近视临床对照研究

【摘要】： 目的比较前弹力层下准分子激光角膜磨镶术(SBK)与准分子激光屈光性角膜切削术(PRK)矫正中低度近视术后对裸眼视力、最佳矫正视力、波前像差、对比敏感度及角膜K结构的影响。 方法对50例(100眼)中低度近视患者随机抽取分别行SBK及PRK术, 7天、1月、3月、6月复查,主要复查指标包括裸眼视力、最佳矫正视力、对比敏感度、波前像差、角膜K结构。 结果视力:术后1周、术后1月SBK组裸眼视力优于PRK组,差别有统计学意义(P0.0001),术后3月、术后6月两组裸眼视力趋于一致(P0.05),两组所有眼裸眼视力均≥0.5,超过90%只眼裸眼视力≥1.0。术后1个月SBK组没有患者最佳矫正视力下降2行,PRK组1(2%)只眼患者最佳矫正视力下降2行。术后3个月、术后6个月两组均没有最佳矫正视力下降2行。屈光度:术后1月SBK组50(100%)只眼屈光度在±1.0D内,44(88%)只眼屈光度在±1.0D内,差别有统计学意义(P0.05)。术后3月、术后6月两组100%只眼屈光度均稳定在±1.0D内。对比敏感度:SBK组与PRK组各频段对比敏感度均较术前下降,差异有显著性(P0.05),6、18c/d频段对比敏感度SBK组高于PRK组,差别有显著意义(P0.05);术后6月:两组对比敏感度均明显恢复,虽低于术前水平但与术前相比差异无显著意义(P0.05).两组之间差异亦无显著意义(P0.05)。波前像差:SBK、PRK术前总的高阶像差、球差、彗差差别均无显著性(P0.05),两组术后总的高阶像差均较术前增大,差异有显著性(P0.01),术后第1月、3月PRK组总的高阶像差均高于SBK组,差异有显著性(P0.05),术后6月两组高阶像差均较术后1月降低,差异有显著性(P0.05),两组之间比较无显著性差异;术后1、3、6月慧差、球差也均较术前增大, SBK组与PRK组相比差异无显著性(P0.05)。角膜K结构:SBK术后1月、3月、6月各时期均可见角膜K结构,PRK术后患者前弹力层缺如,K结构消失。 结论1、SBK组与PRK组两种手术方式矫正中低度近视均有很好的安全性、预测性。 2、术后1月SBK组视力、对比敏感度恢复优于PRK组。 3、术后1、3月SBK组导致的总高阶像差较PRK组低,但两组术后导致的球差、彗差之间无差别。 4、SBK术后K结构的保留与PRK术后K结构的消失为K结构的结构基础位于前弹力层与前基质交界面的特殊纤维排列的假设提供了有力的理论证据。
