双轴线外部标记控制缝线人工晶状体定位精度的临床研究

【摘要】:目的探讨双轴线外部标记技术在睫状沟缝线固定人工晶状体植入术中的定位效果。方法无晶状体眼27例(27眼)接受经巩膜缝线固定人工晶状体植入术,双轴线外部标记技术控制两条子轴线:子轴线1:精确选定等距、对称的巩膜穿刺位点,使角膜中心点投影位于巩膜穿刺位点形成的固定轴线即子轴线1上;子轴线2:精确选定等距、对称的线襻结合位点,使人工晶状体光学中心点位于线襻结合位点形成的固定轴线即子轴线2上。术后随访6~24个月,平均10.8个月。通过测量人工晶状体光学中心点偏位度及超声生物显微镜(ultrasound biomicroscope,UBM)全景测量固定位点轴向人工晶状体倾斜度和与固定位点轴向垂直的轴向人工晶状体偏转度判断人工晶状体位置精度,同时观察可能发生的并发症。结果人工晶状体光学中心点与角膜中心点高度重合者25眼(92.6%),偏离角膜中心点者2眼(7.4%),偏位幅度为(0.23±0.04)mm;UBM全景测量角巩膜缘2-8位点轴向人工晶状体倾斜度为(0.20±0.04)°;UBM全景测量与角巩膜缘2-8位点轴向相垂直的11-5轴向人工晶状体偏转度为(0.24±0.03)°;4眼(14.8%)单襻位于位于睫状沟旁,6眼(22.2%)双襻位于睫状沟旁,人工晶状体光学中心均保持居中;全部病例未发生人工晶状体缝线松解、人工晶状体脱位、角膜失代偿等并发症。结论双轴线外部标记技术具有整体、直观、便捷的技术特征,具备精确控制缝线人工晶状体定位的可行性。
[Abstract]:Objective to investigate the localization effect of double axis external labeling technique in ciliary sulcus suture fixation intraocular lens implantation. Methods 27 cases (27 eyes) of aphakic eyes underwent intraocular lens implantation through scleral suture fixation. Two sub-axis lines were controlled by double-axis external labeling technique: sub-axis 1: accurately select isometric, symmetrical scleral puncture site, so that corneal central point projection was located on the fixed axis 1, which was formed by scleral puncture site. Subaxis 2: accurately select the isometric, symmetrical line loop binding site, so that the optical center point of intraocular lens is located on the fixed axis 2, which is formed by the line loop binding site. The follow-up period was 6 脳 24 months, with an average of 10.8 months. The accuracy of intraocular lens position was judged by measuring the deviation of optical center point of intraocular lens and panoramic measurement of axial intraocular lens inclination at fixed site and axial intraocular lens deflection perpendicular to fixed site by ultrasound biomicroscope,UBM panoramic measurement, and the possible complications were observed at the same time. Results the height of the central point of intraocular lens coincided with the central point of cornea in 25 eyes (92.6%) and deviated from the central point of cornea in 2 eyes (7.4%).
