LASIK术后眼调节幅度的改变及其与视疲劳的关系

【摘要】： 目的：准分子激光角膜屈光手术在我国已有近二十年的发展历程,它的出现为无数的屈光不正患者尤其是近视眼患者摘掉了沉重的眼镜,在人们追求高效率工作与高质量生活的今天,它也成为了很多追求时尚的戴镜人群的一种可以信赖的选择。但是在临床工作中我们发现有部分近视患者在术后早期出现了近距离工作的视疲劳的症状,干扰了正常工作和学习,这应该引起我们的关注。本次研究主要是探讨准分子激光原位磨镶术对近视患者术后单眼及双眼调节幅度的影响,并对调节幅度的变化与视疲劳症状的关系进行探讨,为LASIK术后视觉质量的预测提供一定的临床依据。 方法：入选对象为2009年3月至2009年8月来大连医科大学附属第二医院眼科行LASIK治疗的双眼近视患者年龄在18-35岁之间,共60例120眼,按患者术前近视屈光度分为低中度近视组36例72眼(等效球镜度≤-6.00D),高度近视组24例48眼(等效球镜度-6.00D),正视组为20例40眼(年龄在25.34±5.37,等效球镜度为-0.50-+0.50,BCVA≥1.0)。正视组与近视组分别在术前,术后1w,术后2W,术后1月测定其单眼及双眼调节幅度。分别对正视组和近视组在各个时间对其进行近距工作视疲劳症状的问卷调查。并记录数据。手术均为同一娴熟的医生进行。用SPSS11.0软件对资料进行统计分析,P0.05表示差异有显著意义。 结果： 1.调节功能：所有近视组患者LASIK术后第1周的单眼及双眼调节幅度较术前下降(P0.05),然后逐渐恢复,至术后1月时超过术前水平,且与正视组相比无显著差异。 2.视疲劳症状：近视患者LASIK术后第1周视疲劳症状均较术前明显加重,随时间推移,视疲劳症状逐渐减轻,至术后1月视疲劳症状较术前明显减轻。 3.相关性分析显示：术后1周时调节幅度的降低程度与术前屈光度成正相关；近视组视疲劳症状与双眼调节幅度呈负相关。 结论： 1.LASIK术后患者的单眼及双眼调节幅度均有短暂下降,术后第1周显著下降,以后逐渐恢复。 2.LASIK术后视疲劳症状和调节幅度的变化有关。 3.LASIK手术对近视患者的眼调节幅度未产生不良影响。

It has taken off heavy glasses for countless patients with ametropia, especially myopic patients. Today, when people pursue high efficiency work and high quality of life, it has also become a reliable choice for many people who pursue fashion and wear glasses. However, in clinical work, we found that some myopic patients had symptoms of visual fatigue of close working early after operation, which interfered with normal work and learning, which should attract our attention.
