临床药师参与辅助用药医嘱审核及临床干预效果分析

【摘要】：目的通过临床药师干预,促进我院辅助用药的规范性、安全性和经济性,达到合理用药的目的。方法选取我院2015年2月~2016年1月辅助用药医嘱,对其进行专项点评,将其中发现的典型问题通过临床药师查房宣教的方式加以干预,评价该类药在临床应用中的潜在风险和经干预后的效果。结果我院辅助用药中应用范围最广的是中药注射剂及其衍生物,审核医嘱问题出现较多的分别为丹参川芎嗪注射液、转化糖电解质、脂溶(Ⅱ)/水溶性维生素,问题医嘱比例分别为90.0%、84.6%和83.8%。存在的主要问题是超说明书用法用量、适应证不适宜、联合用药不适宜、溶媒选择不适宜等。结论通过临床药师干预后,我院辅助用药情况得到明显改善,用药更加合理化,从而能够更好地服务于患者。
