河南省基本药物配送模式研究及思考

【摘要】：基本药物制度的实施旨在改变我国传统药品流通环节过长、层层加利的诟病，并且通过对整个医药流通环节各方面利益进行重新调节的方式来实现抑制药价的目标。而基本药物配送作为基本药物制度实施过程中的关键环节，也直接关系着基本药物制度的贯彻实施。因此，基本药物的配送环节具有很重要的研究意义，但是目前国内对基本药物配送环节缺乏一个系统性的规定，且现阶段我国在基本药物配送环节中还存在许多问题亟待解决。本论文通过文献研究、对比研究与实地调研相结合的方法对河南基本药物配送模式进行了研究，继而发现其中存在的问题与不足，借鉴国外先进方法和理念，，力争去探究一个较为科学、合理的配送模式，以解决其中存在的弊端，便于河南及其他省份在不断完善基本药物配送政策时进行参考，也为更好的发展和完善我国基本药物制度打下坚实的基础。本论文主要分为以下几个章节：第一章本论文的研究背景，对基本药物制度、基本药物配送、我国药品流通状况以及新版GSP实施的相关内容进行了概述，此外还对河南省地理、人口、药品流通状况以及本论文的研究意义进行了简单介绍。第二章对各个省份基本药物集中采购及配送模式进行了对比分析，主要包括药物的评标方式、质量层次区分、配送参与模式、药物采购频率及配送时间的要求、相关配送费用和药款支付方式方面的对比，并分析这些差别背后的原因，最后罗列出配送环节中存在的一些问题，就出现问题的深层次原因进行探究。第三章则主要对河南省基本药物配送情况深入介绍，分别从河南省实施基本药物制度的范围、基本药物采购方法及周期、评标方式等内容进行较为详细的阐述，挖掘出河南省基本药物采购中的优点及创新点。第四章首先对基本药物配送成本构成相关内容进行描述，然后将其与我省具体情况相结合设计出了调查表，并向相关企业发放了该问卷。通过汇总、整合这些问卷得出调查结果。最后为探究基本药物配送费用的合理与否，将配送企业与物流公司的配送费用进行了比较。第五章主要探讨了美国、日本、以及欧盟关于医药流通方面的经验。第六章针对河南省基本药物配送过程中存在的问题，在认真分析、总结相关内容并借鉴国外先进经验的基础上，提出了促进我省基本药物配送发展的一些建议，包括加快医药流通企业的整合率；出台相关政策和法律；引入竞争机制以及大力发展第三方物流等建议。第七章是结论部分，对全文进行了总结。
[Abstract]:The implementation of the basic drug system aims to change the traditional drug circulation link in our country, which is criticized by Ghali layer by layer, and to achieve the goal of restraining the drug price by re-adjusting the interests of all aspects of the whole medicine circulation link. The delivery of essential drugs, as a key link in the implementation of the basic drug system, is also directly related to the implementation of the basic drug system. Therefore, the distribution of essential drugs has a very important research significance, but at present there is a lack of a systematic regulation on the distribution of essential drugs in our country, and there are still many problems to be solved in the distribution of essential drugs in our country at present. By means of literature research, comparative research and field investigation, this paper studies the distribution mode of Henan essential drugs, and then finds the existing problems and shortcomings, and draws lessons from foreign advanced methods and concepts. Strive to explore a more scientific, reasonable distribution mode, in order to solve the shortcomings in order to facilitate Henan and other provinces in the continuous improvement of basic drug distribution policy for reference, It also lays a solid foundation for the better development and improvement of China's basic drug system. This paper is divided into the following chapters: chapter one: the research background of this paper, the basic drug system, the distribution of essential drugs, the distribution of drugs in China and the implementation of the new version of GSP related content are summarized. In addition, this paper briefly introduces the geography, population, drug circulation and the significance of this paper. The second chapter makes a comparative analysis of the modes of centralized procurement and distribution of essential drugs in various provinces, mainly including the evaluation of the bid for drugs, the distinction of quality levels, the mode of distribution participation, the frequency of drug procurement and the requirements of distribution time. This paper analyzes the reasons behind these differences, lists some problems existing in the distribution process, and probes into the deep-seated causes of the problems. The third chapter mainly introduces the distribution of essential drugs in Henan Province, respectively from the scope of the implementation of the essential drugs system in Henan Province, the procurement methods and cycles of essential drugs, and the way of bid evaluation, and so on, and so on. The advantages and innovations in the procurement of essential drugs in Henan Province are excavated. The fourth chapter first describes the basic drug distribution cost components, and then designs the questionnaire combined with the specific situation of our province, and issues the questionnaire to the relevant enterprises. Through the summary, integrate these questionnaires to arrive at the results of the survey. Finally, the distribution costs of distribution enterprises and logistics companies are compared in order to explore the rationality of distribution costs of essential drugs. The fifth chapter mainly discusses the experience of the United States, Japan and the European Union on the circulation of medicine. In the sixth chapter, aiming at the problems existing in the distribution of essential drugs in Henan Province, on the basis of careful analysis, summing up the relevant contents and drawing lessons from the advanced experience of foreign countries, some suggestions are put forward to promote the development of the distribution of essential drugs in Henan Province. Including speeding up the integration rate of pharmaceutical circulation enterprises; Introduction of relevant policies and laws; introduction of competition mechanisms and vigorous development of third-party logistics and other recommendations.
