FMEA方法在病区抢救及常备药品质量管理中的应用

【摘要】:目的:规范病区抢救及常备药品质量管理,降低患者的用药风险。方法 :应用失效模式与效应分析(FMEA)方法对病区抢救及常备药品质量管理检查项目和失效模式进行分析,通过对失效模式发生的可能性、严重性和可侦测程度进行评分及风险优先值(RPN)计算,量化并确定应优先改善的失效模式,制订改善措施并实施,并评价实施6个月后的管理效果。结果:确定了包括麻醉及一类精神药品残液处理情况未记录、药品储存温度不达标、药品过期等12项失效模式,制定并实施了相关制度、加强检查管理、冷链管理及日常管理等措施;6个月后,RPN值最高的3项由管理前的320、240、216分分别降至16、16、27分,均处于相对低风险区域中;失效模式事件发生次数由1 869次降为218次,下降了88.3%。结论:我院病区通过应用FMEA方法将抢救及常备药品质量管理过程中失效模式加以量化,确定了风险最高的管理项目并进行改进,显著降低了患者用药风险。
[Abstract]:Objective: to standardize the emergency treatment and the quality management of the standing drugs, and to reduce the risk of drug use. Methods: the failure mode and effect analysis (FMEA) method was used to analyze the items and failure modes of rescue and quality management inspection of standing drugs in ward, and the possibility of failure mode was analyzed. The severity and detection degree were calculated by (RPN), the failure modes that should be improved were quantified and determined, the improvement measures were formulated and implemented, and the management effect after 6 months of implementation was evaluated. Results: 12 failure modes, including unrecorded treatment of anaesthesia and residual liquid of a class of psychotropic drugs, substandard storage temperature of drugs and expiration of drugs, were determined, and relevant systems were formulated and implemented to strengthen inspection and management. Cold chain management and daily management measures; Six months later, the three items with the highest RPN value decreased from 320240216 points before management to 16 points, 16 points and 27 points respectively, all of which were in relatively low risk areas, and the number of failure mode events decreased from 1869 to 218, a decrease of 88.3%. Conclusion: the failure mode in the process of rescue and quality management of standing drugs is quantified by FMEA method in our hospital, and the management items with the highest risk are determined and improved, which significantly reduces the risk of drug use.
