食管癌患者施行PET-CT检查相关人员辐射剂量估算与评价

【摘要】:PET-CT实现了功能成像和解剖显像同机融合,可精确提供靶器官的功能和解剖双重信息,是目前最为先进的影像检查技术之一,尤其在肿瘤分期、疗效评估、良恶性鉴别等方面发挥重要作用。在PET-CT检查流程中,受检者因注射放射性示踪剂会对其本人及其接触者产生辐射危害。~(18)F-FDG是目前临床应用最广的正电子示踪剂,在PET-CT显像中食管癌患者占很大比例,目前对该类患者及相关人群所受的辐射剂量研究尚未见诸报道。本研究追踪食管癌患者~(18)F-FDG PET-CT显像过程,对食管癌患者及相关人员受到的辐射剂量进行估算与评价,以期为临床辐射防护提供参考依据。本研究收集2015年3月~2016年5月在河南省肿瘤医院PET-CT中心行~(18)F-FDG PET-CT显像的食管癌患者100例,男56例,女44例,年龄23~81岁,平均(54.7±14.3)岁。入组标准为经病理确诊且PET-CT显像无转移患者,排除标准为无病理诊断或PET-CT显像有转移患者。100例PET-CT全身显像食管癌患者,利用剂量长度乘积值估算受检者CT扫描所致辐射有效剂量(ED),依据ICRP出版物推荐方法估算受检者~(18)F-FDG所致辐射ED;检测相关人员在不同防护策略下初始剂量当量率(IDR),估算相关人群所受辐射剂量(RD)。本研究结果表明食管癌患者ED为(15.24~26.94)mSv,平均值(20.87±2.06)mSv。在不同防护策略下,全程陪同家属、同病房病友及同病房其他家属每接触1例显像患者RD分别为(26.96~~(18)3.30)μSv、(11.27~45.39)μSv、(5.66~17.84)μSv。基于现有防护策略按每年4500例受检患者,分装护士手部与体部年受照剂量分别为8.96mSv和0.81mSv;注射护士手部与体部年受照剂量分别为13.46mSv和0.63m Sv;技师体部年受照剂量0.9mSv。本论文对食管癌患者行PET-CT检查流程中相关人员在不同防护策略下受照剂量进行了定量分析与研究,以期为临床实践中相关人员的辐射剂量计算与防护优化提供参考依据。
[Abstract]:PET-CT is one of the most advanced imaging techniques, especially in tumor staging, because it can accurately provide the dual information of function and anatomy of target organs by using the same computer fusion of functional imaging and anatomical imaging. It plays an important role in the evaluation of curative effect and differential diagnosis of benign and malignant. It plays an important role in the process of PET-CT examination. At present, F-FDG is the most widely used positron tracer in clinical practice, because radioactive tracer can cause radiation hazard to themselves and their contacts. Esophageal cancer patients accounted for a large proportion in PET-CT imaging. At present, there is no report on the radiation dose of this kind of patients and related population. This study tracks the process of 18F-FDG PET-CT imaging in patients with esophageal cancer. The dose of radiation to esophageal cancer patients and related personnel was estimated and evaluated. In order to provide reference for clinical radiation protection, this study collected and collected from March 2015 to May 2016 at the PET-CT Center of Henan Cancer Hospital. F-FDG PET-CT imaging was performed in 100 patients with esophageal carcinoma. There were 56 males and 44 females, aged 2381 years (mean 54.7 卤14.3) years. The criteria of admission were confirmed by pathology and no metastasis by PET-CT imaging. The exclusion criteria were: no pathological diagnosis or PET-CT imaging with metastasis. 100 patients with esophageal carcinoma by PET-CT systemic imaging. The dose length product value was used to estimate the effective dose of radiation induced by CT scan, and the recommended method of ICRP publication was used to estimate the radiation EDT caused by 18F-FDG. The initial dose equivalent rate (IDR) was measured under different protective strategies. The results of this study showed that Ed was 15.24 ~ 26.94 mSv in patients with esophageal cancer. The average value was 20.87 卤2.06mSv.Under different protective strategies, the family members were accompanied in the whole course. The Rd of one patient in the same ward and other family members in the same ward were 26.96 ~ 183.30) 渭 Svn and 11.27 ~ 45.39 渭 Sv respectively. According to the existing protective strategies, the annual radiation doses of hand and body were 8.96mSv and 0.81mSv, respectively. The annual radiation doses of hand and body were 13.46mSv and 0.63mSv, respectively. The annual radiation dose of technicians was 0.9mSv.In this paper, quantitative analysis and study of radiation dose in the process of PET-CT examination for patients with esophageal cancer under different protective strategies were carried out. In order to provide reference basis for radiation dose calculation and protection optimization in clinical practice.
