职业紧张与尿液中部分激素代谢物水平的关系

【摘要】:目的 职业紧张越来越受到人们的关注,目前国内外评价职业紧张的主要方法是使用心理学量表和问卷,而对职业紧张的客观评价指标研究较少。由于心理学工具的评价结果受主观因素影响较大,因此开展一些职业紧张的客观评价指标的研究就显得尤为重要。 近年来,国内外开展了一些职业紧张客观评价指标的研究,但实验室研究居多,指标介质多为唾液和血液,且得出确定性结论的较少,而以尿液为介质的现场研究开展不多。为此,本次研究增大样本量,开展以随机尿液为指标介质的现场人群研究,旨在了解真实工作条件下,职业紧张及其相关因素与尿液中17-羟皮质类固醇(17-hydroxy-cortico-steroid,17-OHCS)、17-酮皮质类固醇(17-keto-steroid,17-KS)、香草扁桃酸(vanillylmandelic acid,VMA)及高香草酸(homovanillic acid,HVA)水平之间的关系,探讨尿液中17-OHCS、17-KS、VMA及HVA作为职业紧张评价指标的可能性,为职业紧张的识别与评价提供新的科学依据。 方法 以整群抽样的方法,抽取郑州市公安局某派出所的225名在职民警作为研究对象。使用职业紧张量表测试其职业紧张因素、紧张反应、个性特征和紧张缓解因素等。采用酶联免疫吸附试验和高效液相色谱-荧光检测法对尿液中17-羟皮质类固醇(17-OHCS)、17-酮皮质类固醇(17-KS)、香草扁桃酸(VMA)及高香草酸(HVA)水平进行检测。 应用SPSS12.0软件包对人口统计学特征和职业紧张相关因素不同组间的尿液激素代谢物水平差异分别进行t检验或单因素方差分析,以人口统计学特征和职业紧张相关因素为自变量,尿液激素代谢物水平为因变量,进行多因素非条件Logistic回归等统计学分析。检验水准为α=0.05(双侧)。 结果 1人口学特征不同组间激素代谢物水平差异的t检验结果显示,男性、女性的尿液17-OHCS、17-KS、VMA和HVA水平间的差异有统计学意义(均P0.05);饮酒的不同组间比较,17-OHCS、17-KS的水平差异具有统计学意义(均P0.05);不同警种间激素代谢物水平差异比较显示,交警与户籍警、刑警的17-KS水平的差异有统计学意义(均P0.05)。 2紧张相关因素不同评分组间的尿液激素代谢物水平比较结果显示,同事支持、负性情绪的高、低水平组间的17-OHCS水平差异具有统计学意义(t=2.330,P=0.021;t=-2.017,P=0.034);工作保障的高、低水平组间的17-KS水平差异具有统计学意义(t=3.248,P=0.001);同事支持、非工作压力的高、低水平组间的VMA水平差异具有统计学意义(t=3.174,P=0.002,t=-2.319,P=0.022);应付策略的高、低水平组间的HVA水平差异具有统计学意义(t=1.985,P=0.046)。 3以尿液激素代谢物水平为因变量进行多因素非条件Logistic回归分析显示,饮酒和工作控制、尊重均与尿液17-OHCS水平呈负向关系(OR=0.385,P=0.045;OR=0.353,P=0.017;OR=0.352,P=0.045);户籍警、刑警、内在投入、自尊感和每日紧张感与尿液17-OHCS水平呈正向关系(OR=3.409,P=0.018;OR=3.600,P=0.020;OR=3.391,P=0.013;OR=2.439,P=0.041;OR=2.584,P=0.048)。刑警与尿17-KS水平呈正向关系(OR=2.949,P=0.037);工作控制和工作保障均与尿17-KS水平呈负向关系(OR=0.387,P=0.026;OR=0.356,P=0.014)。负性情绪与尿液VMA水平呈正向关系(OR=2.643,P=0.022)。所有人口统计学特征和职业紧张相关因素均未进入以HVA水平为因变量的方程。 结论 工作控制水平低、尊重水平低、内在投入水平高、自尊感水平高和每日紧张感水平高可导致尿液中17-OHCS水平的升高,工作控制水平低和工作保障水平低可导致尿液中17-KS水平的升高,负性情绪水平高可导致尿液中VMA水平的升高;尿液中17-OHCS、17-KS和VMA水平有作为职业紧张客观评价指标的可能性,但需做进一步研究。
[Abstract]:Purpose Occupational stress is getting more and more attention . At present , the main method of evaluating occupational stress at home and abroad is to use psychological scale and questionnaire , and to study the objective evaluation index of occupational stress . As the result of evaluation of psychology tool is influenced by subjective factors , the research on objective evaluation index of some occupational stress is very important . In recent years , there have been studies on objective evaluation indexes of occupational stress at home and abroad , but there are not many laboratory studies , the index media is mostly saliva and blood , and there are few field studies with urine as medium . In order to understand the relationship between the levels of 17 - hydroxy - cortico - steroid ( 17 - OHCS ) , 17 - keto - steroid ( 17 - KS ) , vanillyllic acid ( VMA ) and homovanillic acid ( HVA ) in urine , the paper discusses the possibility of 17 - OHCS , 17 - KS , VMA and HVA in urine as the index of occupational stress assessment , and provides a new scientific basis for the identification and evaluation of occupational stress . method 225 active civilian police officers from a police station of Zhengzhou Public Security Bureau were taken as the subjects . The occupational stress factors , stress responses , personality traits and strain relief factors were tested by using the occupational stress scale . The levels of 17 - hydroxycorticosteroid ( 17 - OHCS ) , 17 - keto corticosteroids ( 17 - KS ) , vanilla - tonine acid ( VMA ) and high - vanillic acid ( HVA ) were tested by enzyme - linked immunosorbent assay and high performance liquid chromatography - fluorescence detection . SPSS 12.0 software was applied to analyze the difference of urine hormone metabolites level between different groups of demographic characteristics and occupational stress . The statistical analysis of demographic characteristics and occupational stress - related factors was carried out . The statistical analysis of multivariate non - conditional logistic regression was conducted . The test level was 伪 = 0.05 ( bilateral ) .
