广西某企业男性工人主动和被动吸烟现状及其对睡眠质量的影响

【摘要】:目的:本课题旨在调查广西壮族自治区某企业的在职男性工人主动和被动吸烟的现状,同时测定其体内的多环芳烃代谢物1-羟基芘的含量,并通过睡眠质量量表调查工人的睡眠质量,从而比较主动及被动吸烟对其睡眠质量的影响。为改善该企业在职男性工人的睡眠状况并提高其生活质量提供参考,同时为制定控制吸烟法律措施提供科学依据。对象和方法:本研究采用的是整群抽样方法,于2015年7月期间在广西某市进行现场调查研究。研究招募到1906名男性工人参与者,调查后,最终得到有效问卷1787份。参与者被收集吸烟情况和被动吸烟情况和人口统计学信息。通过匹兹堡睡眠质量指数表(PSQI)、心理健康状况量表(GHQ-12)与家庭关怀指数量表(Family APGAR index)信息分析参与者的睡眠质量、一般心理健康情况以及家庭功能障碍情况,并用高效液相色谱法测定参与者尿中1-羟基芘的浓度。结果:该企业男性工人主动吸烟率为51.37%,被动吸烟率为31.45%。PSQI总分最高是的主动吸烟组,其次是被动吸烟组。从PSQI单个成分来看,主动吸烟组的工人的睡眠质量、入睡时间、睡眠时间、日间功能障碍的得分均高于其他两组(主动吸烟者被动吸烟非吸烟者)。主动吸烟组和被动吸烟组的工人都能通过入睡时间、睡眠时间、日间功能障碍这三个成分增加工人患睡眠障碍的风险。其中,风险最高的是睡眠时间,被动吸烟组较不吸烟组的风险为1.92倍(OR=1.92 95%CI 1.02-3.63),而主动吸烟组的患病风险高达2.64倍(OR=2.64 95%CI 1.46-4.79)。在纳入二元logistic回归分析模型,调整民族、文化程度、住宅人口、住宅面积、饮酒情况、饮酒频率、锻炼情况、午睡频率、轮班制度、上班天数、上班时间、上班活动、舒张压、心理状况和家庭关怀之后,结果显示,主动吸烟组的睡眠质量风险仍然比不吸烟组的工人风险高出1.41倍(OR=1.41 95%CI 1.05-1.88),被动吸烟组的工人风险高出1.34倍(OR=1.34 95%CI 1.00-1.79)。主动吸烟组尿中的1-OHPyr浓度要高于不吸烟组以及被动吸烟组,其中主动吸烟组尿中1-OHPyr浓度约为不吸烟工人的1.4倍(P=0.007)。此外,尿中高浓度的1-OHPyr与总的睡眠质量呈正相关。结论:本研究发现主动和被动吸烟均能影响男性工人的睡眠质量,其中主动吸烟对男性工人的睡眠质量影响更大。吸烟会导致工人尿中1-OHPyr浓度升高,高浓度的1-OHPyr与总的睡眠质量相关。
[Abstract]:Objective: to investigate the active and passive smoking of male workers in an enterprise of Guangxi Zhuang Autonomous region, and to determine the content of 1-hydroxypyrene, a polycyclic aromatic hydrocarbons metabolite.The effects of active and passive smoking on sleep quality of workers were investigated by sleep quality scale.In order to improve the sleeping condition and the quality of life of male workers in this enterprise, it also provides the scientific basis for making legal measures to control smoking.Objects and methods: a cluster sampling method was used in this study, and a field investigation was conducted in a certain city of Guangxi during July 2015.The study recruited 1906 male workers and ended up with 1787 valid questionnaires.Participants were asked to collect information on smoking and passive smoking and demographics.The participants' sleep quality, general mental health and family dysfunction were analyzed by Pittsburgh Sleep quality Index (Pittsburgh Sleep quality Index), Mental Health status scale (GHQ-12) and Family APGAR Index (Family APGAR Index).The concentration of 1-hydroxy pyrene in urine of participants was determined by high performance liquid chromatography.Results: the active smoking rate of male workers in this enterprise was 51.37. The passive smoking rate was the active smoking group with the highest total score of 31.45%.PSQI, followed by the passive smoking group.According to the single component of PSQI, the scores of sleep quality, sleep time, sleep time and daytime dysfunction in active smoking group were higher than those in other two groups.Both active smoking and passive smoking increased the risk of sleep disorders through three components: sleep time, sleep time, and daytime dysfunction.The highest risk was sleep time. The risk of passive smoking was 1.92 times higher than that of non-smoking group (1.92 95%CI 1.02-3.63), while the risk of active smoking was 2.64 times higher than that of non-smoking group (2.64 95%CI 1.46-4.79).In the dual logistic regression analysis model, adjusted nationality, education, residential population, housing area, drinking, drinking frequency, exercise, siesta frequency, shift system, working days, working hours, working activities, diastolic pressure,After psychological status and family care, the results showed that the risk of sleep quality in active smoking group was still 1.41 times higher than that in non-smoking group, and 1.41 95%CI 1.05-1.88 in passive smoking group, and 1.34 times higher in passive smoking group than 1.34 95%CI 1.00-1.79 in passive smoking group.The concentration of 1-OHPyr in urine of active smoking group was higher than that of non smoking group and passive smoking group. The concentration of 1-OHPyr in urine of active smoking group was about 1. 4 times that of non smoking workers.In addition, high concentrations of 1-OHPyr in urine were positively correlated with total sleep quality.Conclusion: both active and passive smoking can affect the sleep quality of male workers, and active smoking has more influence on the sleep quality of male workers.Smoking leads to an increase in the concentration of 1-OHPyr in the urine, and high levels of 1-OHPyr are associated with overall sleep quality.
