中小学生青春发动时相及影响因素的追踪研究

【摘要】:目的追踪观察郑州市中小学生青春发动时相的现状及发动提前可能的影响因素,并探讨这些因素对青春发动时相影响可能的性别差异,为揭示青春发动时相影响机制提供依据。方法本研究选取了郑州市区3所学校的小学二三年级、初一二三年级女生;小学三四年级、初一二三年级男生,于2014年10月进行有关青春期发育的基线调查,并分别于2015年10月、2016年10月进行后续的追踪调查。此外,本研究还测量了部分样本晨尿中的环境内分泌干扰物的代谢产物。结果1.青春发动时相追踪基期年龄为8岁男生三次调查阴毛发育率依次为0、0、1%,辜丸发育达到Ⅱ期的比例依次为5.9%、12.7%、56.9%;基期年龄为9岁男生三次调查阴毛发育率依次为0%、2.0%、11.0%,睾丸发育达到Ⅱ期的比例依次为8%、17.3%、75.3%;基期年龄为12岁男生三次调查阴毛发育率依次为45.1%、96.1%、100%,睾丸发育达到Ⅱ期的比例依次为88.2%、100%、100%。基期年龄为7岁女生三次调查月经来潮率依次为0%、1.5%、2.3%,乳房发育达到Ⅱ期比例依次为18.2%、48.9%、100%;基期年龄为8岁女生三次调查月经来潮率为0%、0.7%、9.9%,乳房发育达到Ⅱ期比例依次为48.9%、57.4%、80.9%;基期年龄为12岁女生三次调查月经来潮率依次为70.0%、92.0%、97.0%,乳房发育达到Ⅱ期比例均为100%。2.青春发动时相分布追踪三次调查男生青春发动时相提前率依次为17.8%、17.8%、20.3%;三次调查女生青春发动时相提前率依次为12.1%、16.1%、21.7%。3.青春期发育与体格、体脂指标分析男生青春期发育启动年龄与身高、体重、肩胛部皮褶厚度、腹部皮褶厚度之间相关(r=0.609、r=0.343、r=0.209、r=0.170,P值均0.05);女生青春期发育启动年龄与身高、体重、腰围、体脂之间相关(r=0.809、r=0.623、r=0.262、r=-0.254,P值均0.05)。9、10、11、12岁组男生青春期发育启动者身高大于未启动者间;9、10、12岁组男生青春期发育启动者体重大于未启动者。同年龄组女孩青春期发育启动组的身高、体重、腰围、上臂部皮褶厚度、肩胛部皮褶厚度、腹部皮褶厚度的均值均大于未启动组(P值均0.05)。4.青春期发育与体力活动、接触家用化学品等因素分析三次调查男生体力活动得分的均值分别为12.6±5.9、13.8±6.2、14.6±6.2;女生体力活动得分均值分别为11.1±5.0、10.7±4.6、11.5±4.8。三次调查男生日常接触家用化学品的得分均值分别为2.7±1.9、2.2±1.7、2.2±2.5;女生日常接触家用化学品的得分均值分别为2.7±2.0、2.5±1.8、2.6±2.2。三次调查男生接触食品包装材料的得分分别为0.8±1.0、0.8±1.0、0.7±0.9;女生接触食品包装材料的得分情况分别为0.8±1.0、0.7±1.0、0.9±1.0。三次调查男生心理行为发育困难总分分别为7.9±1.8、7.8±1.9、7.9±2.0,亲社会行为得分分别为9.9±4.8、16.4±4.1、16.5±4.1;女生心理行为发育困难总分分别为7.2±2.0、7.5±2.0、7.4±2.2,亲社会行为得分分别为 17.5±4.0、16.9±4.5、16.9±4.9。男女生的体力活动得分、日常接触家用化学品得分、食用食物的食品包装材料得分、晨尿中环境内分泌干扰物、心理行为发育等指标与男女生的青春期发育启动年龄、各年龄男女生青春期发育启动及未启动间未见相关性。结论1.调查期间,同一年龄女孩,乳房发育率高于来潮率。基期调查时,男孩青春发动提前率高于女孩,随访结束时,女孩青春发动提前率高于男孩。2.同一年龄女孩,青春期发育启动者的身高、体重、腰围、上臂部皮褶厚度、肩胛部皮褶厚度、腹部皮褶厚度均比青春期发育未启动者大。3.男女生的体力活动、日常接触家用化学品、食物包装材料、晨尿中环境内分泌干扰物、心理行为发育等指标与青春发动时相与间未发现相关关系。
[Abstract]:Objective To observe and observe the status quo of young students' youth phase and the possible influencing factors of early initiation of youth initiation in Zhengzhou, and to explore the possible gender differences in the influence of these factors on youth initiation, and to provide a basis for revealing the mechanism of the impact of youth starting phase.
