双肺同期大容量肺泡灌洗治疗153例尘肺患者

【摘要】：目的观察双肺同期大容量肺泡灌洗治疗尘肺的疗效及安全性。方法回顾性分析2013年12月至2015年6月于该院行全身麻醉下双肺同期大容量肺泡灌洗的153例患者病例资料,比较手术前后肺功能、血气分析指标、临床评分,观察并发症情况。结果与术前比较,153例患者术后3个月临床评分降低,差异有统计学意义(P0.05);血气分析指标中动脉血氧分压(PaO_2)升高,动脉血二氧化碳分压(PaCO_2)降低,差异有统计学意义(P0.05),而pH值比较,差异无统计学意义(P0.05);肺功能指标中用力肺活量(FVC)、最大通气量(MVV)、第1秒钟用力呼气容积(FEV1)均升高,差异均有统计学意义(P0.05)。无1例严重并发症发生。结论尘肺患者采用双肺同期大容量肺泡灌洗治疗是一种相对无创的病原疗法,其并发症少,安全可靠,可改善患者的临床症状,从而提高其生活质量。
[Abstract]:Objective to observe the efficacy and safety of large volume alveolar lavage in the treatment of pneumoconiosis. Methods from December 2013 to June 2015, 153 patients with double lung and large volume alveolar lavage under general anesthesia were analyzed retrospectively. Pulmonary function, blood gas analysis index, clinical score and complications were compared before and after operation. Results the clinical scores of 153 patients were significantly lower than those before operation (P 0.05), the arterial partial pressure of oxygen (Pao _ 2) in blood gas analysis was increased, and Paco _ 2 (P _ (CO _ 2) _ 2) was decreased, the difference was statistically significant (P < 0.05), and pH value was higher. There was no significant difference (P 0.05), but forced vital capacity (FV), maximal ventilation volume (MVV) and forced expiratory volume (FEV 1) in the lung function index were all increased, and the difference was statistically significant (P 0.05). No serious complications occurred. Conclusion the treatment of pneumoconiosis with large volume alveolar lavage is a relatively noninvasive pathogen therapy with less complications, safety and reliability. It can improve the clinical symptoms and improve the quality of life of pneumoconiosis patients.
