84例“龙虾

【摘要】:目的:总结食用小龙虾所致的横纹肌溶解症的临床特点、治疗及预后。方法:回顾分析本院2016年7月6日至8月26日急诊科收治的84例因进食小龙虾所致横纹肌溶解病例,观察该病的临床特点、治疗及预后。结果:所有患者发病前40 min~24 h均有食用小龙虾史,主要表现为不同程度的肌肉疼痛、肢体无力、呼吸困难及尿色改变等。实验室检测:磷酸肌酸激酶(6 105.49±466.51)U·L~(-1),正常值20~200 U·L~(-1);乳酸脱氢酶(1 080.76±114.80)U·L~(-1),正常值330~445 U·L~(-1);肌红蛋白(571.13±29.38)ng·ml~(-1),正常值25 ng·ml~(-1);所有患者的血肌酐、尿素氮和血钾均在正常范围,部分患者伴有白细胞总数及中性粒细胞比例升高;经碱化尿液、休息、水化、饮水及防治并发症等治疗,所有患者均痊愈出院。结论:哈夫病目前病因、发病机制均不明了,临床表现多样。针对食用小龙虾所致横纹肌溶解患者,及时就诊、早期诊断及治疗,预后较好。
[Abstract]:Objective: to summarize the clinical features, treatment and prognosis of rhabdomyolysis caused by crayfish. Methods: the clinical features, treatment and prognosis of 84 cases of rhabdomyolysis caused by eating crayfish were retrospectively analyzed in the emergency department of our hospital from July 6 to August 26, 2016. Results: all the patients had a history of eating crayfish at 40 min~24 before onset of disease. The main manifestations were muscle pain, limb weakness, dyspnea and urinary color change. Laboratory tests: creatine phosphokinase (6 105.49 卤466.51) U L ~ (-1), lactate dehydrogenase (1 080.76 卤114.80) U L ~ (-1) and lactate dehydrogenase (1 080.76 卤114.80) U L ~ (-1). Myoglobin (571.13 卤29.38) ng ml~ (-1), normal value 25 ng ml~ (-1), serum creatinine, urea nitrogen and serum potassium in all patients were in the normal range, and some patients were accompanied by the increase in the total number of white blood cells and the proportion of neutrophilic granulocytes. After alkali urine, rest, hydration, drinking water and prevention and treatment of complications, all patients were cured and discharged. Conclusion: the etiology and pathogenesis of Haff's disease are not clear at present, and the clinical manifestations are diverse. For the patients with rhabdomyolysis caused by eating crayfish, early diagnosis and treatment have good prognosis.
