大脑中动脉闭塞FLAIR血管高信号征与CTA对比研究

【摘要】：目的探讨FLAIR血管高信号征(FVH)对大脑中动脉闭塞(MCAO)患者软脑膜侧支循环的评估价值。方法对38例发病3天内的MCAO缺血性脑卒中患者的FVH和CTA图像进行对比分析。对FVH行FVH评分(纵向)[FVHS(L)]、FVH评分(横向)[FVHS(T)]两项评定,对CTA重建图像行侧支循环评分(纵向)[CS(L)]、侧支循环评分(横向)[CS(T)]两项评定。比较FVHS(L)与CS(L)、FVHS(T)与CS(T)评分相符程度,并分析FVHS(L)、FVHS(T)、CS(L)、CS(T)之间相关性。结果 38例MCAO患者中,20例(20/38,52.63%)FVHS(L)和CS(L)评价相符,21例(21/38,55.26%)FVHS(T)和CS(T)评价相符。FVHS(L)与CS(L)呈正相关(r=0.48,P=0.002),FVHS(T)与CS(T)呈正相关(r=0.54,P0.001)。FVHS(L)与FVHS(T)呈正相关(r=0.34,P=0.034),CS(L)与CS(T)呈正相关(r=0.56,P0.001)。结论对于MCAO患者,FVH是一种方便、经济、有效的软脑膜侧支循环评估方法。
