5野调强放疗和7野调强放疗的剂量学指标比较及对胰腺癌恶性生物学行为的影响

【摘要】:目的分析5野调强放疗和7野调强放疗的剂量学指标比较及对胰腺癌恶性生物学行为的影响。方法选择在该院接受放疗的胰腺癌患者作为研究对象,随机分为接受5野调强放疗的对照组、接受7野调强放疗的观察组,比较两组患者的靶区及正常组织剂量分布、血清血管内皮生长因子(VEGF)及成纤维细胞生长因子(FGF)水平、血清基质金属蛋白酶(MMP)水平等差异。结果计划靶区(PTV)V110%区、肝脏D1/3、Dmean区、右侧肾脏D1/3 7-filed IMRT低于5-filed IMRT;脊髓Dmax、左侧肾脏D1/2、右侧肾脏D1/37-filed IMRT高于5-filed IMRT(P0.05)。观察组患者接受放疗后的血清VEGF、α-FGF和b FGF水平均明显低于对照组患者(P0.05)。观察组患者接受放疗后的血清MMP-2和MMP-9水平均明显低于对照组(P0.05)。结论 7野调强放疗对于胰腺癌放疗具有放射物理优势,可以有效降低血清VEGF、FGF和MMP等水平,降低胰腺癌恶性生物学行为。
[Abstract]:Objective to compare the dosimetric indexes of 5 field intensity modulated radiotherapy (IMRT) and 7 field intensity modulated radiotherapy (IMRT) and their effects on malignant biological behavior of pancreatic cancer. Methods Pancreatic cancer patients who received radiotherapy in our hospital were randomly divided into two groups: the control group treated with 5 field intensity modulated radiation and the observation group treated with 7 field intensity modulated radiation. The dose distribution of target area and normal tissue were compared between the two groups. The serum levels of VEGF, FGFand MMPs were different. Results the planned target area was V110%, and the D1 / 3 7-filed IMRT of the right kidney was lower than that of the 5-filed, and the D1/37-filed IMRT of the right kidney was higher than that of the 5-filed IMRT P0.05, and the Dmax of the spinal cord was higher than that of the left kidney (D1 / 2), and the D1/37-filed IMRT of the right kidney was higher than that of the 5-filed IMRT (P 0.05). The serum levels of VEGF, 伪 -FGF and b FGF in the observation group were significantly lower than those in the control group (P 0.05). The levels of serum MMP-2 and MMP-9 in the observation group were significantly lower than those in the control group after radiotherapy (P 0.05). Conclusion the 7-field intensity modulated radiation therapy has the advantage of radiophysics for radiotherapy of pancreatic cancer. It can effectively reduce the levels of VEGF and MMP in serum and decrease the malignant biological behavior of pancreatic cancer.
