基于数据挖掘技术分析针灸治疗冠心病心绞痛的经穴运用规律

【摘要】：目的：运用数据挖掘技术分析针灸治疗冠心病心绞痛的古今文献,探索针灸治疗该病的选穴规律和配伍特点。方法：收集针灸治疗冠心病心绞痛的古代文献和现代文献,古代文献收集首先参照林昭庚主编的《中西医病名对照大辞典》,确定“冠心病心绞痛”对应的古代病名,根据课题组前期制定的“古代针灸临床文献采集书目表”,查找和检索所需要录入数据的图书,全面收集相关文献条目。现代文献收集采用计算机检索与手工检索结合的方式,收集以十四经腧穴为主要作用点,以针刺、灸法、电针、激光针等针灸疗法为主治疗冠心病心绞痛的临床报道文献。收集的古今文献经严格筛选,将符合要求的文献录入针灸处方库,进行数据挖掘,分析针灸治疗冠心病心绞痛的腧穴、经络、特定穴运用规律及配伍特点。结果：腧穴运用结果：古代针灸治疗心绞痛共使用腧穴78个,总运用频次271次。频次超过10的腧穴有大陵、上脘、内关、足三里、中脘、曲泽。腧穴配伍结果显示,排在前列的为大陵与内关、大陵与曲泽、中脘与大陵、足三里与上脘、内关与曲泽；所选穴位以四肢及胸腹部穴位为主,配伍中上肢与胸腹部配伍居首。针灸处方中涉及了除手太阳小肠经的13条经络的腧穴。现代针灸治心绞痛共使用腧穴71个,总运用频次565次。使用率居前五的腧穴有内关、心俞、膻中、厥阴俞、神门。腧穴配伍规律的结果显示,排在前列的配伍为内关与心俞、内关与膻中、膻中与心俞、厥阴俞与内关、厥阴俞与心俞；选穴主要集中在四肢与胸腹部、背部腧穴,上肢与背部腧穴配伍居首位。所应用的腧穴广泛分布于十四经。特定穴运用结果：古代针灸处方78个经穴中共使用特定穴63个,占80.77%,特定穴中使用频次最高的为五输穴,其次为交会穴、原穴、募穴、络穴；特定穴配伍关联结果显示,五输穴与原穴、五输穴与络穴、募穴与五输穴、五输穴与交会穴、原穴与络穴配伍频次位居前列。现代针灸处方71个经穴中共使用特定穴61个,占85.92%,特定穴中使用频次最高的为络穴,其次为八脉交会穴、五输穴、背俞穴、交会穴；特定穴配伍关联结果显示,交会穴与背俞穴、络穴与背俞穴、络穴与交会穴、络穴与五输穴、募穴与背俞穴配伍频次位居前列。结论：针灸治疗心绞痛选穴遵循“循经取穴”规律,并注重邻近腧穴及具有经气会聚特点的特定穴的选取。其腧穴配伍方法主要有：本经配穴法,阴经与阴经配穴法,上下配穴法,阴经与阳经配穴法、前后配穴法、局远配穴法、阴经与奇经配穴法、五输穴与原穴配穴法、五输穴与络穴配穴法、募穴与五输穴配穴法、五输穴与交会穴配穴法、原络配穴法、交会穴与背俞穴配穴法、络穴与背俞穴配穴法、络穴与交会穴配八穴法、络穴与五输穴配穴法、俞募配穴法等。
[Abstract]:Objective: to analyze the ancient and modern literature of acupuncture and moxibustion in treating angina pectoris of coronary heart disease by using data mining technology, and to explore the law of acupoint selection and compatibility characteristics of acupuncture and moxibustion treatment. Methods: the ancient and modern literatures of acupuncture and moxibustion on angina pectoris of coronary heart disease were collected. Firstly, referring to Lin Zhaogeng's Dictionary of TCM and Western Medicine, the corresponding ancient names of angina pectoris of coronary heart disease were determined. According to the "Bibliographic Table of Ancient Acupuncture and moxibustion Clinical Literature Collection" made by the research group in the early stage, the books needed to input data were searched and retrieved, and the related literature items were collected. The modern literature collection adopts the combination of computer retrieval and manual retrieval to collect the clinical reports of acupuncture, moxibustion, electroacupuncture, laser acupuncture and other acupuncture therapy for angina pectoris of coronary heart disease. The collected ancient and modern literature was strictly screened, and the documents according to the requirements were entered into the acupuncture and moxibustion prescription database, and the data mining was carried out. The acupoints, meridians, specific acupoints and compatibility characteristics of acupuncture and moxibustion for angina pectoris of coronary heart disease were analyzed. Results: in ancient times, 78 acupoints were used in the treatment of angina pectoris, and the total frequency was 271 times. More than 10 times of acupoints have Daling, Shangwan, Neiguan, Zusanli, Zhongwan, Quze. The results of acupoint compatibility showed that the leading points were Daling and Neiguan, Daling and Quze, Zhongwan and Daling, Zusanli and Shangwan, Neiguan and Quze; Acupuncture and moxibustion prescription involves 13 acupoints of meridians and meridians except the hand-sun small intestine meridian. In modern acupuncture and moxibustion treatment of angina pectoris, 71 acupoints were used, with a total frequency of 565 times. The top five acupoints are Neiguan, Xin Yu, Tanzhong, Jueyin Yu, Shenmen. The results of the law of acupoint compatibility showed that the top compatibility was Neiguan and Xinshu, Neiguan and Shanzhong, Tanzhong and Xinshu, Jueyin Yu and Neiguan, Jueyin Yu and Xinshu; The selection of acupoints is mainly concentrated in limbs and chest and abdomen, back acupoints, upper limbs and back acupoints in the first place. The applied acupoints are widely distributed in the fourteen meridians. The results of application of specific points were as follows: in ancient times, 63 points were used in 78 acupoints, accounting for 80.77 points. The most frequently used points were Wuyou points, followed by rendezvous points, original acupoints, raising points and collaterals. The results showed that the combination frequency of five points and collaterals, three points of raising points and five points of transportation, five points of transportation and rendezvous points, and the frequency of combination of original points and collaterals were in the forefront. The 71 acupoints of modern acupuncture and moxibustion prescription used 61 specific points, accounting for 85.92 points. The points of collaterals and collaterals were the most frequently used in the specific points, followed by the points of eight meridians crossing, the points of Wuyou, the points of backshu and the points of rendezvous. The results of specific points compatibility showed that the cross point and the back Shu point, the collateral point and the rendezvous point, the collateral point and the Wuyou point, and the frequency of the combination of the Fu point and the back Shu point were in the forefront. Conclusion: acupuncture and moxibustion treatment of angina pectoris follow the rule of "taking points along the meridian", and pay attention to the selection of adjacent acupoints and special points with the characteristics of meridian qi convergence. The methods of combination of acupoints are as follows: the method of combining acupoints with the meridian, the method of combining acupoints with yin meridian and yin meridian, the method of combining points up and down, the method of combining points of yin meridian and yang meridian, the method of combining acupoints before and after, the method of matching acupoints by bureau far, the method of combining acupoints by yin meridian and Qi meridian, the method of combining acupoints with original acupoints by five channels, The method of point matching between five points of transportation and collaterals, the method of matching points with points of raising points and five lines of points, the method of matching points with points of rendezvous points and rendezvous points, the method of matching points with points of rendezvous and Bichu points, the method of matching points of collaterals with points of rendezvous. The points of collaterals and Wulu points are combined with acupoints, and the method of Shu-Fu points is used to match points, and so on.
