山西省三级综合医院专科护士使用现状调查与分析

【摘要】:目的:本研究旨在通过问卷调查法,了解山西省三级综合医院专科护士使用现状。探讨并分析影响专科护士使用的主要影响因素,通过对其因素的分析讨论,找出专科护士使用中存在的问题并提出改进方法和策略,完善专科护士主要工作和职责,为促进专科护理的发展,满足社会对专科护士的需求提供有针对性的建议。方法:本研究采用文献研究法拟定专科护士使用情况问卷草案,通过德尔菲法对22名专家进行两轮函询,确定预测问卷。采用整群抽样法,抽取山西省汾阳医院在内的三所三级综合医院专科护士300名作为预调查对象,通过预调查进行条目再修订与筛选,确定最终条目,形成正式问卷。采取整群抽样法对山西省内十所三级综合医院450名专科护士以网络调查方式进行正式调查,采用SPSS20.0软件对数据进行分析、处理。结果:1.专科护士使用情况测评问卷,经探索性因子分析最终构建了由5个维度,37个条目组成的测评工具,其信度Cronbach’a系数为0.964,分半信度是0.743,重测信度是0.936,具有良好的信度和效度。2.对山西省内专科护士的使用现状调查结果显示:专科护士使用情况问卷总分分值较低,专科护士的整体使用情况不理想。各维度使用情况中临床实践得分最高,研究与咨询得分最低。3.经回归分析,培训考核方式、专科护士有无分层使用、有无专科护士职称晋升、绩效考核的政策支持、有无培训考核论文答辩、有无专科护士继续教育培训、培训有无护理人文课程设置对总问卷得分有影响。其中专科护士有无分层使用、有无专科护士职称晋升、绩效考核的政策支持对专科护士使用情况得分有较高的解释力。结论:1.该测评工具具有较好的科学性和实用性,为管理部门了解专科护士工作现状提供了依据,有利于更好的管理专科护士,使其发挥更大的作用。2.山西省内专科护士使用现状不理想,其因素与培训考核方式、医院管理有关。建议规范专科护士的培训与考核,尽快出台专科护士使用与管理方案。
[Abstract]:Objective: the purpose of this study was to investigate the use of specialized nurses in three-level general hospitals of Shanxi province by means of questionnaire survey. This paper probes into and analyzes the main influencing factors on the use of specialized nurses, through the analysis and discussion of the factors, finds out the problems existing in the use of specialized nurses and puts forward the improving methods and strategies to perfect the main work and duties of the specialized nurses. In order to promote the development of specialist nursing and meet the needs of the community for specialist nurses to provide targeted advice. Methods: in this study, a draft questionnaire for the use of specialized nurses was drawn up by literature research, and 22 experts were interviewed twice by Delphi method to determine the predictive questionnaire. Using cluster sampling method, 300 nurses of three tertiary general hospitals in Fenyang Hospital of Shanxi Province were selected as the objects of pre-investigation. The items were revised and screened through pre-investigation, and the final items were determined and a formal questionnaire was formed. A cluster sampling method was used to investigate 450 nurses in 10 tertiary general hospitals in Shanxi Province. The data were analyzed and processed by SPSS20.0 software. The result is 1: 1. By exploratory factor analysis, an evaluation tool composed of 5 dimensions and 37 items was constructed. The reliability of the questionnaire was 0.964, the split-half reliability was 0.743, and the test-retest reliability was 0.936, which had good reliability and validity. The results of investigation on the use status of specialized nurses in Shanxi Province showed that the total score of the questionnaire on the use of specialized nurses was low and the overall use of specialized nurses was not satisfactory. The clinical practice score was the highest and the research and consultation score was the lowest. Through the regression analysis, the training examination way, whether the specialized nurse has the stratified use, whether has the specialized nurse title promotion, the performance appraisal policy support, whether has the training appraisal thesis defense, whether has the specialized nurse continuing education training, Whether the training has the nursing humanities curriculum has the influence to the total questionnaire score. The policy support of performance evaluation has a higher explanatory power to the use of specialist nurses, whether there are stratified use, whether there are professional nurses promotion, and whether the policy support of performance appraisal can explain the use of specialist nurses. Conclusion 1. The evaluation tool is scientific and practical, which provides the basis for the management department to understand the current situation of specialized nurses, is conducive to better management of specialized nurses, so that it can play a greater role. The use of specialized nurses in Shanxi Province is not ideal, and its factors are related to the training and examination methods and hospital management. It is suggested that the training and examination of specialized nurses should be standardized and the use and management scheme of specialized nurses should be introduced as soon as possible.
