个人—组织匹配视角下的云南省住院医师规范化培训满意度研究

【摘要】:住院医师规范化培训制度是国家层面确立的符合行业特点和成长规律的临床医学人才培养制度,也是我国深化医药卫生体制改革和医学教育改革的重大举措。结合当前住院医师规范化培训全面开展以及我国医疗卫生资源不均衡、匹配问题较复杂、住院医师满意度处于低水平的实际,在借鉴国内外相关理论成果的基础上,以云南省某综合性三级甲等医院2014-2016年参加规培的两百余名住院医师作为调查对象,采用自行编制量表进行调查,挖掘住院医师和基地医院两者匹配的影响因素,通过对匹配影响因素和满意度两者相关性进行实证研究,对个人-组织匹配视角下的培训满意度情况做了多维度、全方位的分析比较,结合人口统计学变量进行差异性分析和相关性分析,明确两者匹配影响因素的作用效果,并从住院医师个人层面、基地医院层面提出规范化培训管理策略。结论:(1)不同维度的个人一组织匹配对满意度的一致性作用情况不尽相同。(2)具有不同个体特征变量的住院医师其个人-组织匹配度具有显著性差异。(3)个人一组织匹配度与住院医师的培训满意度存在相关性并呈正相关关系。通过明确住院医师-培训基地医院匹配与培训满意度之间的内在相关性,分别从住院医师个体层面和基地医院组织层面提出住院医师与培训基地医院匹配的合理性理论建议和可行性实践举措,充实住院医师规范化培训管理框架,最终促进住院医师规范化培训有序开展。
[Abstract]:The standardized training system for resident physicians is a clinical medical personnel training system established at the national level, which conforms to the characteristics of the profession and the law of growth. It is also an important measure to deepen the reform of the medical and health system and the reform of medical education in our country. Combined with the fact that the current standardized training of resident doctors is carried out in an all-round way and that the medical and health resources are not balanced in our country, the matching problem is more complex, the satisfaction of resident doctors is at a low level, and on the basis of drawing lessons from relevant theoretical achievements at home and abroad, More than 200 residents of a general Grade 3A hospital in Yunnan Province from 2014 to 2016 were investigated with self-made scale, and the influencing factors of the matching between resident and base hospital were excavated. Through the empirical study on the correlation between matching influencing factors and satisfaction degree, this paper makes a multi-dimensional analysis and comparison of the training satisfaction from the perspective of individual-organization matching. Based on the difference analysis and correlation analysis of demographic variables, the effect of matching influencing factors between them is clarified, and standardized training management strategies are put forward from resident level and base hospital level. Conclusion: (1) the consistency of individual-organization matching in different dimensions on satisfaction degree is different. 2) residents with different individual characteristic variables have significant difference in personal organization matching degree. There was a positive correlation between the degree of matching and the training satisfaction of resident. By clarifying the inherent correlation between the match between resident and training base hospital and the degree of training satisfaction, From the aspect of individual resident and organization of base hospital, the rational theory and feasible practice measures of matching resident and training base hospital are put forward, and the standardized training management framework of resident is enriched. Finally, to promote the standardized training of resident doctors in an orderly manner.
