【摘要】：进入新世纪以来,党和政府大力推进农村医疗保障制度,新型农村合作医疗作为社会主义新农村建设的重要内容已在全国范围内普遍实施。但是,人口众多特别是农村人口众多作为我国现有国情决定了农村合作医疗制度是一项防范程度较低、管理相对粗放、发展很不稳定的制度。因此,新型农村合作医疗制度的有效实施关乎我国农村经济发展和社会稳定大局。农村合作医疗制度自建立以来,在我国广大农村地区经历了几起几落。而理论界对农村医疗保障问题的研究主要集中在：新型农村合作医疗保险制度实施绩效与问题、新型农村合作医疗保险制度的运行机制、进一步完善新型农村合作医疗保险制度等方面,他们的特点都在于紧密结合党和政府的方针政策,联系农村社会保障的进程,寻找问题、分析原因、提出政策与建议,这些研究为指导中国农村医疗保障的改革与发展起了重要作用。但是,这些研究似乎有一定的局限性,确实有进一步研究、补充和发展的必要。本文总结我国农村合作医疗保障在实践过程中的经验教训,结合现阶段的具体国情,针对新型农村合作医疗制度在实施中存在的问题,从农民、政府、制度本身三个方面出发分析原因,并借鉴国际有关农村医疗保障制度的经验,为实现新型农村合作医疗又好又快发展提出建设性的对策与建议,主要包括“隐性”强制与“显性”强制相结合、优化医疗资源配置实现城乡一体化、在农村医疗机构推进基本药物制度、将“共保联办”项目在更大范围内推广等。
[Abstract]:Since entering the new century, the party and the government to vigorously promote the rural medical security system, the new rural cooperative medical system as an important part of the construction of new socialist countryside in the country has been widely implemented. However, a large population especially the rural population as the present situation of our country determines the rural cooperative medical system is a low level of prevention, management relatively extensive development is not a stable system. Therefore, the new rural cooperative medical system is effective implementation of China's rural economic development and social stability.
The rural cooperative medical system since the establishment, in the vast rural areas of China has experienced several ups and downs. And Research on the rural medical security problem the theory focuses on the performance and problems of the implementation of the new rural cooperative medical insurance system, the operation mechanism of the new rural cooperative medical insurance system, to further improve the new rural cooperative medical insurance system and other aspects. Their characteristics is closely combined with the policy of Party and government, with the rural social security process, looking for problems, analyze the causes, and put forward the policy suggestions, these studies to guide the reform and development of rural medical security Chinese plays an important role. However, these studies seem to have certain limitations, have further research and development of the necessary supplement.
This paper summarizes our country rural cooperative medical insurance experience in the practice process, combined with the specific national conditions at the present stage, the new rural cooperative medical system in the implementation of the existing problems in the system itself from farmers, government, the three aspects of the reason analysis, and draw lessons from the international experience about the rural medical security system, put forward the countermeasure and constructive suggestions for the realization of the new rural cooperative medical system development, including the "hidden" mandatory "dominant" and combination of compulsory, optimize the allocation of medical resources to achieve the integration of urban and rural areas, promote the basic drug system in rural medical institutions, the promotion of coinsurance Jo project in a wider range.
