上海市城乡统筹医疗保险制度构建的研究

【摘要】：伴随着上海市城市化水平不断提高,经济水平的不断发展,社会基础保障体系也越发完善。2011年7月,上海市取消了医疗综合保险,将外来从业人员和城郊用人单位从业人员统一纳入到城镇职工基本医疗保险范围,进一步完善优化了上海市整体医疗保险结构体系。上海市目前的基本医疗保险制度以城镇职工基本医疗保险制度、城镇居民基本医疗保险制度为主,但在一些涉农区县实行的是新型农村合作医疗保险制度;另外,城乡医疗救助制度作为整体保障的最后一道防线,这些现行的医疗保险体系对维护城乡贫困人员的基本医疗卫生权益起到了重要作用。然而当前的上海基本医疗保险制度仍然是多元分割运行的体制,存在以户籍标准、就业标准分割运行等情况,部分农民工和灵活就业人员还未被纳入整体覆盖范围,呈现出城乡二元结构和人群分立态势,这在一定程度上制约了上海市社会经济的统筹整体发展和劳动力的自由流动,影响了社会的公平和稳定。因此,统筹城乡基本医疗保险制度的构建是上海市城市整体发展的当务之急,必须尽快设计合理的统筹路径、方式和政策,不断缩小城乡之间、不同群体之间的待遇差距,实现人人公平享有基本的医疗保障。目前,我国总体上已经进入以工促农、以城带乡的发展阶段,已经着力开展破除城乡二元结构、促进城乡一体化发展的重要任务,统筹城乡发展已成为加快推进我国现代化建设和全面建设小康社会的重要战略。上海是我国经济中心,也是农村规模最小的省级行政区。上海具备破除城乡二元结构、促进城乡一体化发展的优越条件和能力,在统筹城乡发展方面,上海处于全国前列,这就要求上海尽快改变城乡分割的基本医疗保险制度,从而促进上海城市社会经济的统筹发展。
[Abstract]:With the increasing level of urbanization and the development of economic level in Shanghai, the social basic security system has become more and more perfect. In July 2011, Shanghai canceled comprehensive medical insurance. Bringing foreign employees and employees of suburban employing units into the scope of basic medical insurance for urban workers, It has further improved and optimized the overall medical insurance structure in Shanghai. At present, the basic medical insurance system in Shanghai consists mainly of the basic medical insurance system for urban workers and the basic medical insurance system for urban residents. However, in some agriculture-related districts and counties, the new rural cooperative medical insurance system is implemented. In addition, the urban and rural medical assistance system serves as the last line of defense for the overall protection. These current medical insurance systems have played an important role in safeguarding the basic medical and health rights of poor people in urban and rural areas. However, the current basic medical insurance system in Shanghai is still a pluralistic and divided operation system, with a household registration standard. Some migrant workers and flexible employment personnel have not been included in the overall coverage, showing a dual structure between urban and rural areas and a trend of separation of people. To a certain extent, this has restricted the overall social and economic development of Shanghai and the free flow of labor, and affected the fairness and stability of the society. The construction of urban and rural basic medical insurance system is an urgent task for the overall development of Shanghai city. It is necessary to design as soon as possible a reasonable overall planning path, methods and policies to continuously narrow the treatment gap between urban and rural areas and between different groups. At present, our country has generally entered the stage of promoting agriculture by industry and taking cities and townships into a stage of development. It has devoted itself to the important task of breaking down the dual structure of urban and rural areas and promoting the development of integration between urban and rural areas. As a whole, urban and rural development has become an important strategy for speeding up the modernization drive and building a well-off society in an all-round way. Shanghai is the economic center of our country and the provincial administrative region with the smallest rural scale. Shanghai has the ability to break down the urban-rural dual structure. Shanghai is at the forefront of the country in terms of promoting the development of urban and rural integration, and Shanghai is at the forefront of the country in coordinating urban and rural development. This requires Shanghai to change the basic medical insurance system of urban-rural segmentation as soon as possible. Thus promoting the overall social and economic development of Shanghai.
