山西省新型农村合作医疗支付方式的改革研究

【摘要】：近年来，“三农”问题一直是中央、国务院重点关注对象，如针对农村实行的减免农业税、九年义务教育等政策，就是要提高农村的经济发展水平和文化教育水平。农村居民的健康问题同样也受到了中央、国务院的重点关注，为此，提出了“新型农村合作医疗制度”，它是新时期国家提高农村卫生服务质量制定的一个大规模政策。新型农村合作医疗制度是农民自愿参加的，并由个人、集体和政府三方筹资，政府起主导作用的制度。在这一制度实施过程中，医疗费用过高过快增长、支付环节管理混乱等，导致农民看病难，看病贵的问题越来越严重，因此要进行一次新农合支付方式的改革来缓解这些问题。所以一个合理的支付方式是非常重要的，不但可以减轻病人的经济负担，，还可以使医疗资源达到最优化，这对新型农村合作医疗制度可持续健康发展，具有十分重要的现实意义。本文主要是通过我国农村医疗保险的发展历程以及支付方式的种类研究，同时借鉴泰国、新加坡、德国等国家的医疗模式，找出适合山西省的最优支付方式，分别从立法、监管、评价、组织管理工作等方面提出了一些对策来完善山西省新农合支付方式改革。
[Abstract]:In recent years, the issue of "agriculture, rural areas and farmers" has been the central government, the State Council has focused on the object of attention, such as the implementation of agricultural tax relief for rural areas, nine-year compulsory education and other policies, is to improve the level of rural economic development and cultural and educational level.The health problems of rural residents have also been paid attention to by the central government and the State Council. Therefore, the "New Rural Cooperative Medical system" is put forward, which is a large-scale policy to improve the quality of rural health services in the new period.The new rural cooperative medical system is a system in which farmers participate voluntarily and are financed by individuals, collectives and governments, and the government plays a leading role.In the process of implementing this system, the medical expenses increase too fast, the management of payment links is confused, and so on, which leads to the problem that it is more and more difficult for farmers to see a doctor and expensive to see a doctor. So we should carry out a reform of the new rural cooperative payment method to alleviate these problems.Therefore, a reasonable payment method is very important. It can not only reduce the economic burden of patients, but also optimize the medical resources. This is of great importance to the sustainable and healthy development of the new rural cooperative medical care system.It has very important practical significance.This article mainly through our country countryside medical insurance development history and the payment way kind research, simultaneously uses for reference Thailand, Singapore, Germany and other countries medical treatment pattern, finds out the suitable Shanxi Province's optimal payment way, separately from the legislation,Supervision, evaluation, organization management and other aspects put forward some countermeasures to perfect the reform of new rural cooperative payment in Shanxi province.
