【摘要】：新型农村合作医疗制度(以下简称“新农合制度”)是一项由政府组织、引导、支持,农民自愿参加,个人、集体和政府多方筹资,以大病统筹为主的农民医疗互助共济制度,是目前我国保障农村居民医疗卫生水平的重要制度。它有利于农民利益得到实现,防止农民因病致贫或因病返贫,对于我国农村医疗卫生事业的发展具有重要的意义。我国的农村合作医疗制度由来已久,经历了从20世纪50年代以来的风风雨雨,经历过兴盛,也经历过衰退,但都没有阻止其前进的步伐。新农合制度的建立是我国各级政府对我国农村卫生事业不断探索与尝试的结果,保证新农合制度的有效实施与可持续发展至关重要,政府作为新农合制度的实施主体责无旁贷。乡镇政府作为与农民最接近的一级政府,对农村与农民的各项情况都相对的熟悉,在新农合制度的实施中发挥着关键作用。乡镇政府在新农合制度的实施中主要承担着新农合政策宣传、组织实施的责任、筹集新农合资金中农民应缴款的责任、协调各方关系、监督管理、保障新农合制度有效实施的责任,乡镇政府应该明确自身在新农合制度实施中的责任。但是,目前乡镇政府在新农合制度的实施中仍然存在责任缺失的问题,制约了新农合制度的发展进程,不利于新农合制度的有效、可持续发展。本文针对新农合制度以及新农合制度实施中乡镇政府的责任缺失问题展开研究,全文主要分为六个章节：第一章绪论,在绪论中阐述了选题的背景及意义,列举了国内外的研究现状,并进一步引出本文所要研究的重点问题,介绍了研究本文的理论依据,从公共服务理论、政府职能理论、公共产品供给理论以及责任政府理论提出了研究本文的依据；第二章主要关于我国农村合作医疗的发展历程展开线索式的介绍,并对新农合制度的建立、内涵、原则、主要特征等进行了分析,为下文的研究提供支持；第三章与第四章,主要从新农合制度实施中乡镇政府承担的具体责任出发,先是乡镇政府应该承担的责任列举,然后从实际情况出发,阐述了新农合制度实施中乡镇政府存在责任缺失的问题,如新农合制度的宣传不到位、资金筹集过程中存在道德风险、新农合制度监管不到位、新农合制度实施中协调沟通不足,以及乡镇政府工作人员的思想意识、管理理念存在问题等,并且进一步分析了乡镇政府责任缺失的原因,如乡镇政府的权责不对称、绩效考核机制不健全、监管机制并不健全等；第五章,主要针对以上提出的乡镇政府在新农合制度实施中的责任缺失以及责任缺失的原因,提出相应的对策,来优化乡镇政府在新农合质实施中的责任,如加大新农合的宣传力度、完善乡镇政府的绩效考核标准、合理划分乡镇政府的事权与财权、提升乡镇工作人员的素质与能力等方面展开阐述。总之,要完善乡镇政府在新农合制度的实施中责任,这需要长时间的不断探索与研究,也需要乡镇政府自身在实际工作中不断的总结经验、反省思考,注重与实施对象的沟通与交流,努力向着更好的方向发展。本文的研究只是关于乡镇政府在新农合制度中责任问题研究的冰山一角,具有很大的局限性,但还是希望能为乡镇政府在新农合制度的实施中更好地履行其责任提供一点参考。
[Abstract]:The new rural cooperative medical system (hereinafter referred to as "the new rural cooperative medical system") is a government organization, guidance, support, the farmers' voluntary participation, individual, collective and government fund-raising, and the farmer's medical mutual aid system based on a large disease overall, is an important system to safeguard the medical and health level of rural residents in China. It is beneficial to the farmers. It is of great significance for the rural medical and health development to realize the realization of the benefits, prevent the farmers from getting poor or to return to poverty in the light of disease. The rural cooperative medical system has a long history, experienced the wind and rain since 1950s, experienced prosperity, and has experienced recession, but it has not stopped the pace of its progress. The establishment of the system of agricultural cooperation is the result of the continuous exploration and trial of China's rural health services at all levels of government. It is essential to ensure the effective implementation and sustainable development of the new rural cooperative system, and the government as the main body of the implementation of the new rural cooperative system is duty bound.
The township government, as the closest government with the peasants, is relatively familiar with the situation of rural and farmers, and plays a key role in the implementation of the new rural cooperative system. In the implementation of the new rural cooperative system, the township government is mainly responsible for the promotion of the new rural cooperative policy, the responsibility of organizing the implementation of the new rural cooperation, and raising the contribution of the farmers to the new rural cooperative funds. Responsibility, coordination of all parties, supervision and management, and the responsibility of ensuring the effective implementation of the new rural cooperative system, the township government should clarify its responsibilities in the implementation of the new rural cooperative system. However, at present, the township government still has the problem of lack of responsibility in the implementation of the new rural cooperative system, which has made the development process of the new rural cooperative system and is not conducive to the new rural cooperative system. This paper is aimed at the study of the lack of responsibility of the township government in the new rural cooperative system and the implementation of the new rural cooperative system. The full text is divided into six chapters: the first chapter is the introduction. In the introduction, the background and significance of the topic are expounded, the status of the research at home and abroad is enumerated, and the key problems to be studied in this paper are further introduced and the research is introduced. The theoretical basis of this article is based on the theory of public service, the theory of government function, the theory of public product supply and the theory of responsible government. The second chapter mainly introduces the clue of the development course of rural cooperative medical care in China, and has carried out the establishment, connotation, principle and main characteristics of the new rural cooperative medical system. Analysis, to provide support for the following research; the third chapter and the fourth chapter, mainly from the implementation of the specific responsibilities of the township government in the implementation of the NCMS system, first of all the responsibilities of the township government should be listed, and then from the actual situation, the problem of the lack of responsibility for the existence of the township government in the implementation of the new rural cooperative system, such as the new agricultural cooperation system, is expounded. The publicity is not in place, there is moral hazard in the process of fund raising, the supervision of the new rural cooperative system is not in place, the coordination and communication of the new rural cooperation system are not enough, the ideological consciousness of the staff of the township government and the problems of the management idea exist, and the reasons for the lack of responsibility for the township government are further analyzed, such as the right and responsibility of the township government is not right. It is said that the mechanism of performance assessment is not sound and the regulatory mechanism is not sound. The fifth chapter mainly aims at the lack of responsibility and the reasons for the lack of responsibility in the implementation of the new rural cooperative system by the township government above, to optimize the responsibility of the township government in the implementation of the new farmers' quality, such as increasing the publicity and improvement of the new rural cooperative system. The performance appraisal standards of township governments, the division of power and financial rights of township governments, and the improvement of the quality and ability of township staff are expounded.
In a word, it is necessary to improve the responsibility of the township government in the implementation of the new rural cooperative system. It needs a long time to explore and study, and it also needs the township government to constantly summarize the experience in the actual work, reflect on it, pay attention to the communication and communication with the implementation object, and try to develop in a better direction. This study is only about the township government. The iceberg in the study of the responsibility of the government in the new rural cooperative system has great limitations, but it still hopes to provide a reference for the township government to better perform its responsibility in the implementation of the new rural cooperative system.
