城乡医疗保障一体化背景下苏州市参保人医疗服务满意度及就医行为研究

【摘要】：伴随着我国经济的不断发展和新医改的持续深入,我国的医疗保障体系建设取得了较快发展。苏州市凭借其强大的经济实力和对医疗保障体系的不懈探索,于2012年率先实现了城乡医疗保障一体化,理论上说来这一政策对于实现苏州市城乡居民就医的可及性和公平性、提高对医疗服务总体的满意度、促成三级医疗机构患者就医的合理分流、乃至推动整个江苏省的城乡医疗保障一体化进程都具有重大的借鉴意义。那么这一改革在实施两年多来是否实现了上述预期的效果?是否对于江苏省其他地区、乃至全国各个地区都具有借鉴意义?这些都有待于实际的调查与探讨。本研究主要以苏州市城乡医疗保障一体化为背景,以苏州市吴江区的500名参保人为研究对象,采用问卷调查的方式,从个人基本信息、参保人对新政策的认知程度、参保人对新政策的满意度、参保人的就医行为及其忠诚度等多维度进行问卷的设计与调查,对问卷的分析和处理主要采用描述性统计、单因素分析和多元回归方程等统计方法,以此了解城乡医疗保障一体化之后苏州市参保人对医疗服务满意度及就医行为的变化。通过分析,我们发现苏州市对卫生资源的投资在逐年增加,满足了更多参保人的卫生需求,城乡一体化的医疗保障制度实现了城乡居民就医的可及性与公平性,从整体上提高了参保人对医疗服务的满意度,这些都充分验证了城乡医疗保障一体化政策的重大意义。但也存在着诸如尚未实现参保人就医选择的合理分流,城乡一体化医疗保障政策的宣传力度有待加强等问题。
[Abstract]:With the continuous development of economy and the deepening of new medical reform, the construction of medical security system has made rapid development. Suzhou, with its strong economic strength and unremitting exploration of the medical security system, took the lead in realizing the integration of urban and rural medical security in 2012. Theoretically, this policy has contributed to the realization of the accessibility and fairness of access to health care for Suzhou's urban and rural residents. It is of great significance to improve the overall satisfaction of medical services, to promote the rational diversion of patients in tertiary medical institutions, and even to promote the integration of urban and rural medical security in Jiangsu Province. So has this reform achieved the desired results over the past two years or so? Is it useful for other regions of Jiangsu Province, and even for all regions of the country? All these need to be investigated and discussed in practice. This study mainly takes the integration of urban and rural medical security in Suzhou as the background, takes 500 participants in Wu Jiang district of Suzhou as the research object, adopts the method of questionnaire, from the personal basic information, the insured's cognition to the new policy. The participants' satisfaction with the new policy, the medical behavior and loyalty of the insured are designed and investigated. The statistical methods of descriptive statistics, single factor analysis and multivariate regression equation are used to analyze and deal with the questionnaire. In order to understand the integration of urban and rural health care security after Suzhou City to the medical service satisfaction and behavior changes. Through analysis, we find that the investment in health resources in Suzhou is increasing year by year, which meets the health needs of more and more participants, and the integrated medical security system of urban and rural areas has realized the accessibility and fairness of access to health care for urban and rural residents. As a whole, it improves the satisfaction of the insured to the medical service, which fully verifies the significance of the integration policy of urban and rural medical security. However, there are still some problems, such as the reasonable diversion of the insured's choice of medical treatment, the propaganda of the urban-rural integrated medical security policy and so on.
