湖南省新型农村合作医疗保障制度的现状及对策

【摘要】：建立农村医疗保障系统,有助于国家政权向农村深入、有利于解决农村的医疗需求问题。新型农村合作医疗制度是由政府组织、引导和支持的农民医疗互助共济制度,研究和完善新型农村合作医疗制度,具有重大战略意义。本文选取了湖南省具有代表性的岳阳地区,分析湖南省推行新型农村合作医疗制度所取得的成绩和面临的问题,并结合所发现的问题和不足制定出相应的解决方案,旨在为湖南省新型农村合作医疗制度进行决策和完善时提供理论依据和参考意见。结果如下：明确政府职责、强化健全管理机制。针对新型农村合作医疗制度设计存在的弊端,指出政府应建立和完善政府的责任机制,将农村合作医疗的责任落到实处,确保各级政府做到真正意义上的高度重视,重点组织,用心实施,确保农村合作医疗的成功。新农合医疗制度法制化。针对新型农村合作医疗确保基金顺畅运行和消除参与主体的疑虑,要尽快实现合作医疗制度法制化,一方面能够保障农民筹资工作法制化、规范化,也赢取农民群众的信赖。全力提升乡村医疗队伍的服务能力。针对地区间和行业内医疗队伍服务水平和行为的差异,指出须提高新型农村合作医疗制度建设的经费投入,提高农村医院人才引进和人才培养的资金投入,提高农村医院医用设备的物质投入。加强建设面向贫困农民的医疗救助制度。针对起付线和封顶上线不合理、补偿比例偏低等情况,应采取保大必须保小的补偿机制,扩大农民的受益面,充分利用医疗基金。因此,未来湖南省农村合作医疗制度的发展要呈现以下特点,即举办法制化、参与社会化、形式多样化、管理规范化、城乡一体化,并通过以下几个方面推进和完善农村合作医疗制度：建立健全法制政府责任约束机制以明确地方各级政府责任、加强宣传和引导以吸引农民积极参与农村合作医疗、调整和完善合作医疗个人筹资机制、建立合理的补偿机制以完善合作医疗制度的基础与效用。
[Abstract]:The establishment of rural medical security system will help the state power to go deeper into rural areas and solve the problem of rural medical needs. The new rural cooperative medical care system is organized, guided and supported by the government. It is of great strategic significance to study and perfect the new rural cooperative medical care system. This paper selects the representative Yueyang area of Hunan Province, analyzes the achievements and problems faced by Hunan Province in the implementation of the new rural cooperative medical system, and combines the problems and shortcomings found out to formulate a corresponding solution. The purpose of this paper is to provide theoretical basis and reference for the decision-making and perfection of the new rural cooperative medical system in Hunan Province. The results are as follows: Clear government responsibility, strengthen and perfect management mechanism. In view of the defects in the design of the new rural cooperative medical care system, it is pointed out that the government should establish and perfect the government's responsibility mechanism, carry out the responsibility of the rural cooperative medical care, and ensure that the governments at all levels attach great importance to it in a real sense, and focus on the organization. We will increase the material input of medical equipment in rural hospitals. We will strengthen the construction of a medical aid system for poor farmers. In view of the unreasonable starting line and top line, and the low proportion of compensation, we should adopt the compensation mechanism which must protect the small, enlarge the benefit area of farmers and make full use of medical fund. Therefore, the future development of the rural cooperative medical system in Hunan Province should show the following characteristics: organizing legalization, participating in socialization, diversification of forms, standardization of management, integration of urban and rural areas, And through the following aspects to promote and improve the rural cooperative medical system: establish and improve the legal system of government responsibility constraint mechanism to clarify the responsibilities of local governments, strengthen publicity and guidance to attract farmers to actively participate in rural cooperative medical care, Adjusting and perfecting the individual financing mechanism of cooperative medical system and establishing reasonable compensation mechanism to perfect the foundation and utility of cooperative medical system.
