新型农村合作医疗条件下衡阳县农村老人合作医疗制度完善研究

【摘要】：农村老龄化问题已成为一个广受社会关注的问题，这个问题不仅直接影响着我国的经济、政治、与文化因素，也给农村的养老体系提出了巨大的挑战，这个问题已经成为政府难以回避的重大问题。近年来，医疗费用呈现出逐年上升的趋势，农民“看不起病，吃不起药”的现象非常普遍，很多农民由于经济原因的影响均不愿入院接受治疗，农民缺乏医疗的保障，，看病支出均是自己负担，这就给家庭带来了巨大的经济压力，与城市居民相比而言，农民也面临着更多的疾病隐患，新型农村合作医疗保险制度正是基于此而产生，该种制度是由政府指导、农民自愿参与，政府、集体、个人共同筹资的一种医疗互助制度。虽然新型农村合作医疗保险制度在衡阳县已经实施了一段时间，但是依然难以为老年人群体提供全方位的保障。由于各种因素的影响，自我养老与家庭养老模式还存在着各种不足，因此，社会保险途径也会是解决农村老年人养老问题的主要途径，为了完善新型农村合作医疗保险制度，需要明确政府责任，加大扶植力度、加强监督与管理，促进保险基金的增值、加强宣传，提升农民尤其是老年人的参保意识、完善法律制度，营造出制度环境、转变农村经济发展模式，促进农村经济的良性循环，本文正是基于此来分析新农合下农村老人合作医疗制度的完善措施。本文分五章阐述了新农合下农村老人合作医疗制度的完善方式，第一章深入的介绍了国内外研究现状与选题意义，得出我国现行的医疗保险体系在补偿比例、制度设计、筹集方式、资金规模和管理主体等方面都存在着一定的差异，城乡之间存在着较大的差别，在基金收入与待遇方面，城镇居民医疗保险是新型农村合作医疗人均筹资额的十倍左右，整体的制度有待进一步完善。第二章分析了新型农村合作医疗制度基本原则以及新型农村合作医疗制度的相关理论，这为本文的研究奠定了坚实的理论技术。第三章分析了新农合下农村老人合作医疗制度中存在的问题，得出，这些问题有些属于制度设计的范畴，有些属于政策制定的范畴，有些属于配套措施的问题，这些问题均严重影响了农村老人合作医疗制度的正常执行。第四章采用问卷调查的形式分析了衡阳县新型农村合作医疗制度的执行情况，得出无论从经济水平还是从健康状况来分析，农村老人均称为社会弱势群体，其中，疾病是影响农村老年人生活质量的主要因素，农村老年人在患病后，在就诊机构的选择上主要为村卫生室、个体诊所与乡镇卫生院，虽然中央与地方上增加了对卫生院的资金投入，但是地方财政往往难以及时的落实，这就导致农村卫生院的建设工作滞后，而从总体而言，新农合农村老人合作医疗制度还存在种种问题。第五章是本文的核心，主要根据新农合下农村老人合作医疗制度中存在的问题提出相应的解决措施。本文的研究得出，由于各种因素的影响，自我养老与家庭养老模式还存在着各种不足，因此，社会保险途径也会是解决农村老年人养老问题的主要途径，为了完善新型农村合作医疗保险制度，需要明确政府责任，加大扶植力度、加强监督与管理，促进保险基金的增值、加强宣传，提升农民尤其是老年人的参保意识、完善法律制度，营造出制度环境、转变农村经济发展模式，促进农村经济的良性循环。
[Abstract]:The problem of rural aging has become a social concern , which not only directly affects the economic , political and cultural factors of our country , but also poses a great challenge to the old - age system in the countryside . In recent years , the medical expense has become a major problem that the government has difficulty to avoid . In recent years , the farmers are not willing to accept treatment because of the economic reasons . The new rural cooperative medical insurance system is a kind of medical mutual aid system which is led by the government and the farmers are voluntarily involved , the government , the collective and the individual . Although the new rural cooperative medical insurance system has been implemented in HengYang county for some time , it is still difficult to provide all - round guarantee for the elderly population .

Because of the influence of various factors , the self - supporting and the family pension pattern still exist , so the social insurance way is the main way to solve the old - age pension problem in the rural areas . In order to perfect the new rural cooperative medical insurance system , it is necessary to clarify the government responsibility , strengthen the promotion , strengthen the supervision and management , promote the value - added of the insurance fund , build up the system environment , transform the rural economic development mode , and promote the virtuous circle of the rural economy .

In the first chapter , the author gives a detailed introduction to the current situation of medical insurance system in the countryside and the significance of choosing the subject , and concludes that the present medical insurance system has some differences in the proportion of compensation , the system design , the way of raising funds , the scale of funds and the subject of management .

The second chapter analyzes the basic principles of the new rural cooperative medical system and the relevant theories of the new rural cooperative medical system , which lays a solid theoretical technique for the study .

The third chapter analyzes the problems existing in the cooperative medical system of the rural old people under the new agriculture , and concludes that some of these problems belong to the scope of the system design , some belong to the category of the policy formulation and some problems which belong to the matching measures , which seriously affect the normal execution of the rural elderly cooperative medical system .

The fourth chapter analyzes the implementation of the new rural cooperative medical system in HengYang county by means of questionnaire , and concludes that the rural old people are called the social vulnerable groups , whether from the economic level or the health status , among which the disease is the main factor affecting the quality of life of the rural elderly .

The fifth chapter is the core of this paper , and puts forward the corresponding measures according to the problems existing in the cooperative medical system of rural old people under the new agriculture .

In order to perfect the new rural cooperative medical insurance system , it is necessary to clarify the government ' s responsibility , strengthen the strengthening , strengthen the supervision and management , promote the value - added of the insurance fund , strengthen the propaganda , promote the farmers , especially the old people , perfect the legal system , create the institutional environment , transform the pattern of rural economic development , and promote the virtuous circle of the rural economy .
