农民工基本医疗保险的缴费意愿研究

【摘要】:缴费意愿是提高农民工基本医疗保险参保率的重要标准。农民工医保缴费意愿越强烈,参保积极性也就越高,也能够整体上提高我国农民工基本医疗保险的覆盖率。我国农民工在城市从事的多数是环境较差、条件艰苦、劳动时间长、疾病风险较大的工作,对自身健康消耗极大。然而,多数农民工缺乏健康维护意识,对基本医疗保险的认识程度也不够深入,大多以消耗健康来赚取收入,不利于农民工长久的劳动生产。现阶段,农民工的整体覆盖率较低,我国医保制度区域分割和城乡分割的特点,又阻碍了农民工获取相应的医疗保障待遇,这显然和我国社会保障制度发展的目标相违背。因此,我们有必要对农民工的医保缴费意愿进行研究,以提高农民工基本医疗保险的参保率和保障水平,推动全国统一劳动力市场建设。本研究首先对选题的研究背景和意义进行了阐释,回顾了国内外学者对农民工基本医疗保险缴费意愿研究的文献综述,并介绍了本文的理论基础,即Grossman健康需求理论和福利经济学理论,然后对本文关键名词进行了概念界定。其次,文章梳理了农民工基本医疗保险的政策历程,对农民工的医保参保现状和缴费能力进行分析,并立足于访谈法研究影响农民工医保缴费能力的潜在因素。再次,本文立足于实地调研数据,实证研究了农民工基本医疗保险缴费意愿的影响因素,重点考察的是医保报销方便程度和健康变量对农民工医保缴费意愿的影响。最后,文章基于实证研究结果,提出了政策建议。本文研究发现,我国农民工基本医疗保险的参保现状为整体参保率低、同时参保率存在行业和区域差异;农民工的收入和消费水平与医保缴费能力存在关联性。且实证研究结果表明,医疗保险报销程度越方便显著提高了农民工的缴费意愿;健康状况越差则农民工的缴费意愿越低;接受过职业培训的农民工有较强的缴费意愿;农民工的收入和医保缴费意愿呈现显著的正相关关系;工作环境越差、转换城市工作次数越多则缴费意愿越低;家庭耕地面积越多的农民工其缴费意愿越大。为了更好提高农民工的医保缴费意愿,增强农民工的医保缴费能力,本文建议采取以下措施:第一,建立统一的基本医疗保险异地报销制度。可建立区域间医保联网系统、建立规范的医保信息管理体系、加快城乡基本医疗保险一体化建设,以推动统一的基本医疗保险异地报销制度建设。第二,强化对外出农民工的健康教育。政府部门应建立农民工健康教育培训班,农民工个人应努力学习健康知识,社会舆论应加大全民健康宣传,推动建立人人都维护健康的社会大环境。第三,加强财政对农民工基本医疗保险个人缴费部分的补助。政府应该对困难农民工个人缴费实行补助,进而促进困难农民工享有医疗保障待遇。第四,设立农民工基本医疗保险多种缴费档次。将医保缴费按照一定的档次进行划分,能够让农民工获得足够的选择空间,应进一步落实企业的缴费责任,扩大覆盖面,建立缴费档次转接制度,从而有效保障农民工的医疗保障权益。
[Abstract]:The willingness to pay the fee is an important standard to improve the insurance rate of basic medical insurance for migrant workers. The stronger the willingness of migrant workers to pay for medical insurance, the higher the enthusiasm of the migrant workers, the higher the coverage rate of the basic medical insurance for migrant workers in our country. However, the majority of migrant workers lack the awareness of health maintenance, and the understanding of basic medical insurance is not deep enough. Most of them make income from the consumption of health, which is not conducive to the long-term labor production of migrant workers. At this stage, the overall coverage rate of peasant workers is low, and the regional division of medical insurance system in China is divided. The characteristics of cutting and urban and rural division have hindered the migrant workers from getting the corresponding medical treatment, which is obviously contrary to the goal of the development of social security system in our country. Therefore, it is necessary to study the willingness to pay for medical insurance for migrant workers so as to improve the insurance rate and the level of insurance for the basic medical insurance of migrant workers, and to promote the national unified work. This study first explains the research background and significance of the topic, reviews the literature review of scholars at home and abroad on the willingness to pay for basic medical insurance for migrant workers, and introduces the theoretical basis of this paper, namely the Grossman health demand theory and the theory of welfare economics, and then the key nouns of this paper have been carried out. Secondly, the article combed the basic medical insurance policy of migrant workers, analyzed the status of medical insurance participation and payment ability of migrant workers, and based on the interview method to study the potential factors affecting the medical insurance payment ability of migrant workers. Again, this paper, based on the field survey data, empirically studied the basic medical insurance payment of migrant workers. The influence factors of the willingness to pay are focused on the effect of the convenience of medical insurance reimbursement and the influence of health variables on the willingness to pay for medical insurance of migrant workers. Finally, based on the results of the empirical research, the article puts forward the policy suggestions. The income and consumption level of migrant workers are related to the payment ability of medical insurance. The results of the empirical study show that the more convenient the medical insurance reimbursement is, the better the willingness to pay for the migrant workers; the worse the health condition is, the lower the willingness of the migrant workers to pay. There is a significant positive correlation between the income of migrant workers and the willingness to pay for medical insurance; the worse the working environment, the more the conversion of urban work, the lower the willingness to pay, the greater the willingness to pay for the more rural migrant workers in the family. In order to better improve the willingness to pay for medical insurance and enhance the medical insurance payment ability of migrant workers, this paper suggests that the migrant workers' medical insurance payment ability will be enhanced. The following measures are taken as follows: first, establish a unified basic medical insurance foreign reimbursement system, establish an interregional medical insurance network system, establish a standardized medical insurance information management system, accelerate the integration of urban and rural basic medical insurance, in order to promote the construction of a unified basic medical insurance reimbursement system. Second, strengthen the health of migrant workers. Education. Government departments should set up a training class for health education for migrant workers. The individual should work hard to learn health knowledge. Public opinion should increase public health publicity and promote the establishment of a healthy social environment for all. Third, strengthen the financial subsidy to the personal contribution of basic medical insurance for migrant workers. The personal payment of migrant workers should be subsidized to promote the medical insurance treatment for the migrant workers. Fourth, the basic medical insurance of migrant workers is set up. The medical insurance payment can be divided in accordance with a certain grade, which can make the migrant workers get enough space. We should carry out the payment responsibility of the enterprises, expand the coverage and establish the coverage. The transfer system of payment grades can effectively protect the rights and interests of medical insurance for migrant workers.
