城居保与家庭医疗消费支出负担:政策效应评估——基于工具变量方法与稳健性检验

【摘要】：本文基于国家基本医疗保险入户调查数据,利用工具变量法,在解决模型内生性的基础上,从家庭医疗消费绝对支出和相对支出两个维度,探究了城居保对家庭医疗消费支出负担的影响。研究发现城居保总体上释放和扩展了居民的卫生服务利用,城居保显著提升了家庭医疗消费绝对支出水平,平均增加约1500元/年,同时,也显著提升了家庭医疗消费相对支出水平,平均提升近3个百分点。此外,研究发现城居保对城市户籍、非低保户、慢病户三类家庭的绝对和相对医疗消费支出均有显著提升作用。
[Abstract]:Based on the national basic medical insurance household survey data, using the instrumental variable method, on the basis of solving the endogenous model, from the two dimensions of household medical consumption absolute expenditure and relative expenditure, This paper explores the impact of urban housing insurance on the burden of household medical expenditure. The study found that CZP has generally released and expanded the utilization of health services for residents, and CZP has significantly increased the absolute level of household medical expenditure, with an average increase of about 1500 yuan per year. At the same time, It also significantly raised the relative level of household health care spending, by an average of nearly 3 percentage points. In addition, it is found that CZP can significantly improve the absolute and relative medical consumption expenditure of urban household registration, non-low insured households and chronic families.
