新型农村合作医疗制度绩效研究

【摘要】：新型农村合作医疗制度自2003年试点、推广至今已十年有余。这十年里,新农合制度的发展之快大家有目共睹,同时在缓解农民因病致贫、因病返贫,以及增强农民疾病经济风险防御能力方面也立下了不少功绩,其发展可谓在“速度”与“质量”两方面齐头并进。但尽管如此,随着我国社会经济飞速发展,人们对物质文化等各方面需求急速上升,显然当前的新型农村合作医疗制度还不能满足农村居民日益增长的医疗需求。并且,在农村医疗保障制度发展的同时,城镇医疗保障制度也以更快的速度发展得愈加完善,城乡居民间的医疗保障水平仍然存在较大差距。在此背景下,进一步完善新型农村合作医疗制度,实现其可持续发展,对促进社会公平,缩小城乡差距,协调城乡发展,构建和谐社会具有重大意义。本论文的研究目的在于考察新型农村合作医疗制度的实施绩效,分析其存在的主要问题,以期为进一步完善新农合制度做出应有贡献。本文构建了以考察新农合对农民健康、收入及就医流向三方面影响的新农合制度绩效评价框架,采用适用于公共政策绩效评估的双重差分计量模型对这三方面影响进行了估计,并对实证结果进行了相应分析。最后,结合实证分析结果,提出了加快经济增长,提高农民收入；进一步完善现行新农合制度；加大监督力度,确保基金运行效率和安全；以及进一步加大投入,改善基层医疗卫生条件等政策建议。
[Abstract]:The new rural cooperative medical system has been popularized for more than ten years since it was piloted in 2003. In the past decade, the rapid development of the new rural cooperative system has been obvious to all, and at the same time, it has also made a lot of achievements in alleviating the poverty caused by illness, returning to poverty because of illness, and enhancing the ability of farmers to defend themselves against disease economic risks. Its development can be described as "speed" and "quality" two aspects go hand in hand. However, with the rapid development of social economy in our country, people's demand for material culture and other aspects is rising rapidly. Obviously, the current new rural cooperative medical system can not meet the growing medical needs of rural residents. Moreover, with the development of rural medical security system, the urban medical security system has also developed more and more perfect at a faster rate, and there is still a big gap in the level of medical security between urban and rural residents. In this context, it is of great significance to further improve the new rural cooperative medical system and realize its sustainable development to promote social equity, narrow the gap between urban and rural areas, coordinate urban and rural development, and build a harmonious society. The purpose of this paper is to investigate the implementation performance of the new rural cooperative medical system and analyze its main problems in order to make due contributions to the further improvement of the new rural cooperative medical system. In this paper, a new rural cooperative system performance evaluation framework is constructed to investigate the effects of NCMS on farmers' health, income and medical flow, and the double difference measurement model suitable for public policy performance evaluation is used to estimate the impact of these three aspects. The empirical results are analyzed accordingly. Finally, combined with the results of empirical analysis, this paper puts forward to speed up economic growth and improve farmers' income; further improve the current new rural cooperative system; strengthen supervision to ensure the efficiency and safety of fund operation; And further increase investment, improve primary medical and health conditions and other policy recommendations.
