【摘要】：本论文首先从提升医疗服务业绩效的制度安排：理论框架。围绕医疗运营效率提高以及医疗服务质量改善的医疗服务业绩效提升问题，通过比较医院绩效提升的内部方法和外部方法，强调了外部方法对提升医院绩效的特殊作用。进而针对外部方法中的市场和管制对医院的激励和约束作用，阐明市场与管制间的关系。最后从所有权对医疗服务业绩效的激励作用，以及市场与管制间的权衡全面分析提升医疗服务业绩效的制度框架。特别强调了管制对医疗服务业绩效提升的局限性，指出应该通过提高医疗服务市场透明度增加医疗服务市场的运行效率，，以及通过实施反应灵敏型的管制提高管制效率。发达国家提升医疗服务业绩效的制度安排：以美国和英国为例。世界各国在建立了确保公平的医疗保险体系后，均面临如何提升医疗服务业绩效问题。因为医疗服务业绩效提升是确保公众长期享受医疗服务公平的支撑。围绕医疗服务业绩效提升时确保公众长期享受医疗服务公平的支撑。其中以美国和英国最具有代表性。通过对美国和英国为提升医疗服务业绩效的医疗改革实践的梳理，总结出可以为中国借鉴的经验。中国提升医疗服务业绩效的制度安排。在对中国医疗改革的演变历史进行简单回顾的基础上,基于所有权对医疗服务业绩效的激励作用，研究建立符合中国实际的医院所有权格局和公立医院法人治理结构，进而设计市场与管制相融合的有利于提升医疗服务业绩效的模式及其制度框架。最后提出了我国提升医疗服务体系的良性的运作制度“社区看门人”。
[Abstract]:Firstly, this thesis starts with the institutional arrangement of improving the performance of medical service industry: theoretical framework. Focusing on the improvement of medical operation efficiency and the improvement of medical service quality, this paper compares the internal and external methods of hospital performance improvement, and emphasizes the special role of external methods in improving hospital performance. Furthermore, the relationship between market and regulation is clarified in view of the incentive and restraint effect of market and regulation on hospital. Finally, the institutional framework of improving the performance of medical service industry is analyzed comprehensively from the incentive effect of ownership on the performance of medical service industry and the tradeoff between market and regulation. The limitation of regulation on the performance of medical service industry is emphasized, and it is pointed out that the efficiency of regulation should be improved by improving the transparency of medical service market and by implementing responsive regulation. Institutional arrangements for improving the performance of Medical Services in developed countries: take the United States and the United Kingdom as examples. After establishing a fair medical insurance system, countries all over the world are faced with the problem of how to improve the performance of medical service industry. Because the health service industry performance improvement is to ensure the public long-term health care service fair support. Around the performance improvement of the medical service industry to ensure that the public enjoy the health care service equity support for a long time. Among them, the United States and Britain are the most representative. By combing the practice of health care reform in the United States and Britain to improve the performance of the medical service industry, the experiences that can be used for reference by China are summarized. China's institutional arrangements for improving the performance of medical services. On the basis of a brief review of the evolution history of China's medical reform, and on the basis of the incentive effect of ownership on the performance of medical service industry, this paper studies the establishment of hospital ownership pattern and corporate governance structure of public hospitals in line with the actual situation in China. Furthermore, the paper designs the model and institutional framework of the integration of market and regulation to improve the performance of medical service industry. Finally, it puts forward the healthy operation system of improving medical service system in our country, community gatekeeper.
