基于两周患病率的南京市未成年人医疗需求研究

【摘要】：未成年人医疗保障是我国医疗保障体系的重要组成部分，随着社会的不断发展，家庭也日益呈现出核心化的特征，未成年人的医疗保障己经成为影响未成年人生存发展权利、家庭生活乃至于我们社会稳定的制度性问题。而目前我国未成年医疗保障制度亟待建立和完善。对于作为国家未来希望的未成年人来说，这样的情况明显不利于我国未来的发展。本文以南京市作为研究对象，利用大量详实的数据，对南京市未成年人医疗需求进行分析研究并分析影响南京市未成年人医疗保障需求的因素。进一步的，对南京市未成年人医疗保障制度的设计提出更加有效率的改进：即应针对未成年人的年龄，未成年人家庭收入，未成年人父母最高学历等因素设计多层次的未成年人医疗保障制度。由于未成年人医疗保障制度有很强的公共产品色彩，故本文在开篇介绍了我国目前相关的政治、经济背景。在政府关注民生和医疗改革的情况下，我国的经济发展状况，尤其是收入差距较大，城乡家庭收入差距较大的情况下，我国对于未成年人医疗保障的普及和完善的要求是迫切的。并对已有的研究进行总结，，介绍文章的脉络。第二章，即理论回顾部分，用供给需求曲线、帕累托最优和公共产品理论对未成年人医疗保障进行分析研究。第三章，对南京市未成年人医疗保障现状的分析：（1）对南京市目前的医疗资源配置现状进行分析，发现南京市目前存在着资源分配不均，过于集中于城区的现象，且医疗资源无法满足南京市未成年人的医疗需求；（2）分别介绍南京市城镇家庭和农村家庭的未成年人医疗保障现状及其存在的不足之处。第四章，对南京市未成年人医疗需求及其影响因素做理论上的分析，以《南京市统计年鉴》与《南京市卫生统计年鉴》等的相关数据为出发点，分析南京市城乡家庭未成年人医疗的需求及其影响因素。第五章，以《南京统计年鉴》、《南京卫生统计年鉴》及现场调查数据对南京市未成年人医疗需求进行了实证分析，采用χ^2检验和Logistic回归模型来对南京市未成年人医疗需求的影响因素进行分析，南京市未成年人的医疗保障需求影响因素的影响大小的排序为：年龄父母最高文化程度家庭年收入家庭住址所在地。文章的最后是对本文的总结，并针对本文得到的结论提出部分政策建议。通过研究得出以下主要结论：第一，目前南京市医疗资源有限且呈现出分布不均衡的现象，城乡的医疗保障制度均未针对未成年人设计，只是从全民的角度，在一定程度上满足了未成年人的医疗需求；第二，南京市未成年人医疗需求仍属于刚性需求，且受到其自身健康状况、经济、家庭社会的影响；第三，在建立和完善未成年人医疗保障制度时，应综合考虑多种因素，建立多层次的保障制度
[Abstract]:The medical security of minors is an important part of the medical security system of our country. With the continuous development of the society, the family also presents the characteristic of core day by day. The medical security of the minors has become the right to influence the survival and development of the minors. The institutional problems of family life and even our social stability. But at present our country minor medical security system urgently needs to establish and consummates. For the minors who hope for the future of our country, this situation is obviously unfavorable to the future development of our country. This article takes Nanjing as the research object, uses a large amount of detailed data, carries on the analysis research to the Nanjing minor medical treatment demand and analyzes the factor which affects the Nanjing minor medical security demand. Furthermore, the design of the medical security system for minors in Nanjing is proposed to be more efficient: that is, it should be directed at the age of minors and the family income of minors. Parents of minors and other factors such as the design of multi-level medical care system for minors. Because the medical security system of minors has a strong color of public goods, this article introduces the political and economic background of our country at the beginning of the article. With the government paying close attention to the people's livelihood and the health care reform, the economic development situation in China, especially when the income gap is large, the income gap between urban and rural households is large, The requirement of popularizing and perfecting medical security for minors in our country is urgent. The existing research is summarized and the context of the article is introduced. The second chapter, the review of the theory, uses the supply demand curve, Pareto optimal and public goods theory to analyze and study the medical security of minors. The third chapter analyzes the status quo of medical security for minors in Nanjing: (1) analyzes the current situation of medical resources allocation in Nanjing, and finds that there is an uneven distribution of resources in Nanjing, which is too concentrated in urban areas. And the medical resources can not meet the medical needs of minors in Nanjing. (2) introduce the status quo and shortcomings of the medical security of the urban and rural families in Nanjing. The fourth chapter makes a theoretical analysis of the medical needs of minors in Nanjing and its influencing factors, taking the relevant data of Nanjing Statistical Yearbook and Nanjing Health Statistics Yearbook as the starting point. This paper analyzes the needs and influencing factors of medical treatment for minors in urban and rural families in Nanjing. The fifth chapter, with Nanjing Statistical Yearbook, Nanjing Health Statistics Yearbook and field survey data, the medical needs of minors in Nanjing are analyzed empirically. Using 蠂 ^ 2 test and Logistic regression model to analyze the influencing factors of the medical needs of minors in Nanjing. The order of the influencing factors of the medical security demand of the minors in Nanjing is as follows: the location of the home address of the annual income of the parents with the highest education level. The last part of the article is a summary of this paper, and some policy recommendations are put forward. The main conclusions are as follows: first, at present, the medical resources in Nanjing are limited and the distribution is not balanced. The medical security system in urban and rural areas is not designed for minors, but only from the perspective of the whole people. To a certain extent, it meets the medical needs of minors. Second, the medical needs of minors in Nanjing are still rigid and affected by their own health status, economy and family society. Third, When establishing and perfecting the medical security system for minors, we should consider a variety of factors and establish a multi-level security system.
