软骨ADAMTS-7基因上调与股骨头坏死发生之间的相关性研究

【摘要】:目的探讨ADAMTS-7与ONFH(股骨头坏死)患者关节软骨退化和影像学变化的相关性,并研究其潜在机制。方法共纳入2014年3月至2015年6月我院收治的80例股骨颈骨折(FNF)患者作为研究对象。根据检查结果将所有患者分为对照组和ONFH组,对照组患者X线片检查均无ONFH和骨关节炎OA;ONFH组患者X线片检查有明确ONFH。根据ARCO分期标准,将ONFH组患者进一步分为两组:A组,ARCO分期II~III期,X线片显示股骨头无明显塌陷;B组,ARCO分期IV期,X线片显示股骨头有明显塌陷。记录Mankin评分;免疫组化染色分析ADAMTS-7、TNF-α和磷酸化NF-κB的表达水平;RTPCR检测ADAMTS-7的mRNA水平;Western Blot检测ADAMTS-7与COMP的蛋白水平;分析各组指标差异,探讨ADAMTS-7在ONFH发病机制中的潜在作用。结果各组患者的年龄、性别等基本信息对比无统计学差异(P0.05)。ONFH组患者关节软骨发生明显退化,ONFH A组与ONFH B组的Mankin评分均显著高于对照组(P0.05)。ONFH组的ADAMTS-7和TNF-α表达水平均显著高于对照组(P0.05)。ONFH组ADAMTS-7的mRNA水平显著高于对照组(P0.05)。Western Blot结果显示ADAMTS-7的增加与COMP的降解潜在相关(P0.05)。此外,ONFH组患者的NF-κB表达水平显著高于对照组(P0.05)。结论 ONFH患者关节软骨ADAMTS-7、TNF-α与磷酸化NF-κB均显著上调,ADAMTS-7的表达水平与软骨TNF-α和磷酸化NF-κB P65的表达呈正相关,ADAMTS-7可能通过TNF-α与磷酸化NF-κB P65参与ONFH的发病机制。
[Abstract]:Objective to investigate the correlation between ADAMTS-7 and the degeneration of articular cartilage and imaging changes in patients with osteonecrosis of the femoral head (ONFH). Methods 80 patients with femoral neck fracture treated in our hospital from March 2014 to June 2015 were included in the study. All the patients were divided into control group and ONFH group according to the results of the examination. There was no ONFH in the control group and no ONFH in the osteoarthritis group. According to the standard of ARCO staging, the X-ray examination of the patients in the control group had definite ONFH. The patients in ONFH group were further divided into two groups: group A and group A were divided into two groups: II~III stage X ray film showed that there was no obvious collapse of femoral head in group B and group B showed obvious collapse of femoral head in stage IV of ARCO stage. Mankin score was recorded. The expression of ADAMTS-7 TNF- 伪 and phosphorylated NF- 魏 B was analyzed by immunohistochemical staining. The mRNA level of ADAMTS-7 and the protein level of ADAMTS-7 and COMP were detected by RT-PCR and the protein levels of ADAMTS-7 and COMP were detected by Western Blot, and the difference of indexes was analyzed to explore the potential role of ADAMTS-7 in the pathogenesis of ONFH. There was no significant difference in sex and other basic information. The ADAMTS-7 and TNF- 伪 expression levels in ONFH group and ONFH B group were significantly higher than those in control group P0.05. ONFH group were significantly higher than that in control group P0.05. ONFH group. ADAMTS-7 in ONFH group was significantly higher than that in control group P0.05. ONFH group. The expression of ADAMTS-7 and TNF- 伪 in ONFH group were significantly higher than those in control group (P 0.05) and in ONFH group (P 0.05). The level of mRNA was significantly higher than that of control group (P 0.05). The results of Western Blot showed that the increase of ADAMTS-7 might be associated with the degradation of COMP. In addition, the expression of NF- 魏 B in patients with ONFH was significantly higher than that in control group. Conclusion the levels of ADAMTS-7 TNF- 伪 and phosphorylated NF- 魏 B in articular cartilage of ONFH patients were significantly higher than those in control group. The expression of ADAMTS-7 was positively correlated with the expression of TNF- 伪 and phosphorylated NF- 魏 B p65 in cartilage. ADAMTS-7 may be involved in the pathogenesis of ONFH through TNF- 伪 and phosphorylated NF- 魏 B p65.
