前列腺干细胞抗原基因rs2294008位点多态性与东亚人群胃癌罹患风险的Meta分析

【摘要】：目的:采用Meta分析的方法评估前列腺干细胞抗原(prostate stem cell antigen,PSCA)基因rs2294008 CT位点多态性和东亚人群罹患胃癌风险的相关性。方法:计算机检索PubMed、Web of Science、Cochrane Library、Google学术、中国知网、中国生物医学文献数据库、维普和万方等各大数据库,收集建库开始至2016年6月6日公开发表的关于PSCA基因rs2294008位点多态性与东亚人群罹患胃癌风险的病例-对照研究。按照纳入及排除标准筛选文献,进行质量评价后提取数据。采用STATA 14.1软件进行Meta分析,计算合并比值比(odds ratio,OR)及95%可信区间(confidence interval,CI),并进行亚组分析和Meta回归分析以评估混杂因素,最后进行敏感性分析和发表偏倚检测。结果:共纳入13项病例-对照研究,包含13 667例胃癌患者和11 868例健康对照。Meta分析显示,与其他基因型相比,东亚人群携带PSCA rs2294008CT和TT基因型时罹患胃癌的风险明显增高(OR=1.68,95%CI:1.43~1.96);PSCA rs2294008多态性在中国、日本和韩国人群之间存在显著的分布差异(P=0.001)。亚组分析提示,携带PSCA rs2294008 CT和TT基因型的中国人群(OR=1.36,95%CI:1.23~1.49,P=0.004)、日本人群(OR=2.61,95%CI:2.22~3.07,P=0.001)和韩国人群(OR=2.04,95%CI为1.28~3.27,P=0.012)罹患胃癌的风险比其他基因型均明显增高;按对照来源为医院和社区进行的亚组分析显示,各组别罹患胃癌的风险无明显差异(OR=1.74,95%CI:1.23~2.44;OR=1.64,95%CI:1.40~1.93)。Meta回归分析显示,国家和对照组来源的差异不是纳入研究之间异质性的主要来源(P=0.163,P=0.381)。Begg检测未发现发表偏倚(校正P=0.053),敏感性分析确认上述结果具有稳定性(OR=1.54,95%CI:1.45~1.64)。结论:PSCA rs2294008 CT基因多态性与东亚人群罹患胃癌的风险密切相关。
[Abstract]:Objective: To evaluate the correlation of the rs2294008 CT site polymorphism of the prostate stem cell antigen (prostate stem cell antigen, PSCA) gene and the risk of gastric cancer in East Asia by Meta analysis. A case-control study on the polymorphism of the PSCA gene rs2294008 locus and the risk of gastric cancer in East Asia, published in June 6, 2016, was collected from the large databases such as universal and Wanfang. The data were extracted after quality evaluation according to the inclusion and exclusion criteria, and the Meta analysis was carried out by STATA 14.1 software. Combined ratio Ratio (odds ratio, OR) and 95% confidence interval (confidence interval, CI), subgroup analysis and Meta regression analysis were used to evaluate confounding factors. Finally, sensitivity analysis and publication bias detection were carried out. Results: a total of 13 case control studies were included, including 13667 cases of gastric cancer and 11868 healthy control.Meta analysis, and The risk of gastric cancer in the East Asian population with PSCA rs2294008CT and TT genotype was significantly higher than other genotypes (OR=1.68,95%CI:1.43~1.96), and there was a significant distribution difference between the PSCA rs2294008 polymorphism in China and the Korean population (P=0.001). The subgroup analysis suggested that the Chinese people carrying PSCA rs2294008 CT and TT genotype were in China.
