楚雄彝族自治州103株结核分枝杆菌基因分型及耐药性研究

【摘要】:目的探究楚雄彝族自治州流行的结核分枝杆菌基因分型情况与耐药的相关性。方法收集2014年4月-2015年4月在楚雄分离得到结核分枝杆菌菌株,采用比例法检测6种抗结核药物的敏感性,采用间隔区寡核苷酸分型法(Spoligotyping)进行基因分型。结果本研究共分离获得103株结核分枝杆菌临床分离株,其总体耐药率和耐多药率分别为15.33%、6.80%。Spoligotyping分型可分为北京基因家族(Beijing family)75株(72.82%)及非北京基因家族(NonBeijing family)28株(27.18%)2个大基因群,共包括18个基因型,含6个基因簇及12个独立基因型,4个独立基因型为本研究新发现基因型。北京基因家族中,典型北京基因型70株(93.33%,70/75)为主要的流行株;非北京基因家族中,T1家族13株(46.43%,13/28)为主要的流行株。北京基因家族与非北京基因家族总体耐药率分别为17.33%(13/75)、10.71%(3/28),二者差异无统计学意义(χ~2=0.270,P=0.603);耐多药率分别为8%(6/75)、3.57%(1/28),二者差异无统计学意义(χ~2=0.126,P=0.723)。结论北京基因家族为楚雄彝族自治州流行的优势菌株,与非北京基因家族的耐药性无统计学差异。
[Abstract]:Objective to investigate the relationship between the genotyping of Mycobacterium tuberculosis and drug resistance in Chuxiong Yi Autonomous Prefecture. Methods Mycobacterium tuberculosis strains were isolated from Chuxiong from April 2014 to April 2015. The susceptibility of 6 antituberculous drugs was detected by proportional method and genotyping by Spoligotyping. Results A total of 103 clinical isolates of Mycobacterium tuberculosis were isolated. The total drug resistance rate and multidrug resistance rate were 15.33 and 6.80 Spoligotyping, respectively. There were 75 strains (72.82%) of Beijing family and 28 strains (27.18%) of nonBeijing family. There were 18 genotypes, including 6 gene clusters and 12 independent genotypes, and 4 independent genotypes were the new genotypes found in this study. In the Beijing gene family, 70 typical Beijing genotypes (93.33 / 70 / 75) were the main epidemic strains, and 13 (46.43 / 28) were the main epidemic strains in the non-Beijing gene families. The overall drug resistance rates of Beijing gene family and non-Beijing gene family were 17.33% (13 / 75) and 10.71% (3 / 28), respectively. There was no significant difference between them (蠂 ~ (2 / 2) 0.270) and multidrug resistance (8% (6 / 75) or 3.57% (1 / 28), respectively). Conclusion the Beijing gene family is the dominant strain in Chuxiong Yi Autonomous Prefecture, and there is no significant difference in drug resistance between Beijing gene family and non-Beijing gene family.
