活血散瘀汤联合围刺治疗寻常型斑块状银屑病的临床观察

【摘要】：目的:以中医学理论为依据,观察"活血散瘀汤"联合围刺疗法治疗寻常型斑块状银屑病(血瘀证)的临床疗效,以皮损面积及严重程度指数(PASI)评分和中医瘙痒症候评分作为评价指标,进行观察和分析。方法:将符合入选标准的90名斑块状银屑病患者采用随机分组的方法分为治疗组、对照组,每组各45人。观察过程中脱落4例,实际为治疗组42例、对照组44例。治疗组给予口服"活血散瘀汤"配合围刺疗法及对照组给予口服"血府逐瘀胶囊",两组均联合外用"凡士林",共治疗56天。通过观察皮损面积及严重程度指数(PASI)评分变化及瘙痒症候程度,分别于治疗后第28天、第56天记录评分并进行疗效判定,以此评价"活血散瘀汤"联合围刺疗法的临床疗效。并对患者在治疗期间出现的不良反应进行评估,从而对药物的安全性进行评价。结果:1.治疗第28天,两组各自于治疗前、后相比较,PASI评分和中医瘙痒症候评分均存在显著性差异(P0.05);治疗第56天末,两组间比较,上述两指标评分均存在显著性差异(P0.05)。2.治疗第56天,以PASI评分的改善程度作为疗效评判指数,治疗组42例患者中临床痊愈1例、显效24例、有效11例、无效6例,总有效率85.71%;相比治疗组,对照组44例患者中临床痊愈0例、显效1 6例、有效1 7例、无效11例,总有效率75.00%。两组疗效比较存在显著性差异(P0.05)。结论:1."活血散瘀汤"联合围刺疗法与"血府逐瘀胶囊"联合"凡士林"相比,前者在减少银屑病患者PASI分值及缓解主观中医瘙痒症候上优于后者,疗效确切,且安全性高。2."活血散瘀汤"联合围刺是治疗寻常型斑块状银屑病血瘀证的有效方法。
[Abstract]:Objective: to observe the clinical effect of "Huoxue Sanyu decoction" combined with peri-acupuncture therapy on plaque psoriasis vulgaris (blood stasis syndrome) based on the theory of traditional Chinese medicine. The index of lesion area and severity (PASI) and the score of pruritus in traditional Chinese medicine (TCM) were used as the evaluation index. Methods: 90 patients with plaque psoriasis were randomly divided into treatment group (n = 45), control group (n = 45) and control group (n = 45). 42 cases in the treatment group and 44 cases in the control group. The treatment group was treated with "Huoxue Sanyu decoction" and the control group was given "Xuefu Zhuyu capsule", both of which were combined with "vaseline". Total 56 days of treatment. By observing the lesion area and severity index (PASI) and the degree of pruritus symptoms, the scores were recorded on the 28th day and the 56th day after the treatment and the curative effect was evaluated. In order to evaluate the clinical efficacy of "Huoxue Sanyu Tang" combined with peri-acupuncture therapy, and to evaluate the adverse reactions of patients during treatment, the safety of the drug was evaluated. Results: 1. The 28th day of treatment. Before and after treatment, there were significant differences between the two groups in PASI score and TCM pruritus score. At the end of the 56th day of treatment, there was significant difference between the two groups in the scores of the above two indexes. On the 56th day of treatment, the improvement degree of the PASI score was taken as the evaluation index of the curative effect. In the treatment group, 1 case was clinically cured, 24 cases were markedly effective, 11 cases were effective, 6 cases were ineffective. The total effective rate was 85.71%. Compared with the treatment group, there were 0 cases of clinical cure, 16 cases of remarkable effect, 17 cases of effective and 11 cases of ineffectiveness in the control group (44 cases). The total effective rate was 75.00. There was a significant difference between the two groups. Conclusion: 1. "Huoxue Sanyu Tang" and "Xuefu Zhuyu capsule" combined with "vaseline" were compared. The former is better than the latter in reducing PASI score and relieving subjective pruritus in psoriasis patients. The combination of Huoxue Sanyu decoction and Peripheral needling is an effective method for the treatment of blood stasis syndrome of plaque psoriasis vulgaris.
