左归丸对子宫内膜异位症术后GnRH-a副作用的治疗及生存质量的影响

【摘要】：目的:观察左归丸对子宫内膜异位症术后的患者应用GnRH-a所致副作用的治疗情况及患者生存质量的影响。方法:所有入组的患者术后给予国产醋酸亮丙瑞林微球3.75mg一针,每隔4周注射一次,共3针,在第一针治疗后出现潮热汗出、失眠等副作用的肾阴虚型患者,随机分成对照组及试验组。对照组30例,在第二针时反向添加戊酸雌二醇1mg/d及醋酸甲羟孕酮2mg/d。试验组30例,在对照组的基础上口服左归丸每日2次。观察Kupperman评分、EHP-5中文版的评分量表,监测雌二醇情况。结果:使用一针后两组E2水平均处于低雌激素水平,两组比较差异无统计学意义(P0.05),使用3针时两组E2水平比较差异无统计学意义(P0.05)。改良Kupperman评分:(1)试验组和对照组的患者治疗4周后,两组的评分与术前相比较,差异有统计学意义(P0.05)。两组治疗12周与治疗4周前后比较差异有统计学意义(P0.05)。(2)治疗12周时两组比较差异有统计学意义(P0.05)。治疗4周及停药3月后两组评分比较无差异(P0.05)。EHP-5评分:(1)两组治疗12周与治疗4周前后比较,差异有统计学意义(P0.05)。(2)治疗12周时两组在症状影响生活、自我印象、性生活情况方面比较差异有统计学意义(P0.05)。(3)停药后3月试验组和对照组两组比较差异无统计学意义(P0.05)。结论:(1)使用左归丸是安全有效的。(2)内异症术后的患者使用GnRH-a时,兼口服左归丸可以明显减轻GnRH-a的副作用。(3)左归丸能够提高患者的生存质量,降低患者的负性心理,让使用GnRH-a类药物的患者适应此阶段治疗,提高患者的依从性。
[Abstract]:Aim: to observe the effect of Zuogui pill on the side effects of GnRH-a and the quality of life of patients with endometriosis after operation. Methods: all the patients in the group were treated with a single injection of 3.75mg every 4 weeks and 3 injections. After the first injection, the patients suffered from the side effects of hot flashes, sweating, insomnia and other side effects, such as deficiency of kidney yin. They were randomly divided into two groups: control group and experimental group. 30 patients in the control group were treated with estradiol valerate (1mg/d) and medroxyprogesterone acetate (2mg / d). The experimental group (30 cases) was treated with Zuo Gui pill twice a day on the basis of control group. To observe the Kupperman score and EHP-5 score scale in Chinese version, and monitor the status of estradiol. Results: the level of E _ 2 in the two groups was at low estrogen level after one injection. There was no significant difference between the two groups in E _ 2 level, but there was no significant difference in E _ 2 level between the two groups after the use of three needles. After 4 weeks of treatment, the scores of the two groups were significantly different from those before treatment (P 0.05). There was significant difference between the two groups before and after treatment at 12 weeks and 4 weeks after treatment (P 0.05. 05) at 12 weeks of treatment, there was a significant difference between the two groups (P 0. 05). There was no difference in the scores between the two groups after 4 weeks of treatment and 3 months after withdrawal. There was no significant difference between the two groups in terms of symptoms affecting life and self impression at 12 weeks after treatment compared with those before and after treatment for 12 weeks and before and after 4 weeks of treatment. There was no significant difference between the experimental group and the control group 3 months after the withdrawal of the drug, there was no significant difference between the two groups (P 0.05). Conclusion (1) it is safe and effective to use Zuogui Pill. (2) when patients with endometriosis are treated with GnRH-a, the side effects of GnRH-a can be significantly alleviated by taking Zuogui Pill.) Zuo Gui Pill can improve the quality of life of patients and reduce the negative psychology of the patients. Make patients with GnRH-a drugs adapt to this stage of treatment and improve compliance.
