【摘要】: 【目的】:经翼点手术入路的应用越来越广泛,迫切需要对大脑中浅静脉的类型、汇入点及其吻合静脉进行显微解剖研究。 【方法】:本实验采用15例经福尔马林固定的成人尸体标本头颅,用红色乳胶灌注颅内动脉,蓝色乳胶灌注颅内静脉。将头皮沿正中矢状线切开并将其翻向两侧,沿一侧眶上缘、关键点、颧弓上缘和枕外隆突的连线及矢状缝取下一侧颅盖。打开硬脑膜,依次分离各区的桥静脉,并对其进行体表定位。观察并测量各区外侧面静脉的多项指标如管径、条数等。按同样的方法处理对侧半球,在手术显微镜下观察测量30侧大脑浅静脉及其吻合静脉形态。 【结果】:大脑中浅静脉表现为单干型(10侧)、双干型(11例)、三干型(6例)和未发育型(3例)。上吻合静脉(Trolard静脉)和下吻合静脉(Labbe静脉)是大脑浅静脉主要吻合静脉,它们都与大脑中浅静脉相联系。并不是所有的大脑半球吻合静脉都发达。Trolard静脉左侧半球缺如4例,右侧半球缺如9例。Labbe静脉左侧半球缺如4例,右侧半球缺如3例。 【结论】:在神经外科手术中,,熟练掌握吻合静脉的显微解剖对判断术中静脉损伤能否引起的术后并发症非常重要,静脉能保留的尽可能保留。经翼点入路分离外侧裂时,应熟练掌握大脑中浅静脉的显微解剖。在吻合静脉不发达的病例中损伤大脑中浅静脉会引起手术并发症。
[Abstract]:[Objective]: The pterion approach is applied more and more widely, the urgent need of the superficial middle cerebral vein, and into the vein anastomosis on the microsurgical anatomy.
[method]: this experiment used 15 formalin fixed adult cadaveric head specimens perfused with red latex and intracranial arteries, blue latex perfusion of intracranial venous. The scalp incision along the sagittal line and turn it to two sides, one side along supraorbital margin, key points, zygomatic arch and sagittal margin and on line the external occipital protuberance suture to remove the calvarium. Open the dura, followed by separation of the bridging veins of the District, and on its positioning. Observe and measure the lateral vein district a number of indicators such as diameter, number and so on. The contralateral hemisphere by the same method, observed and measured on 30 sides of the superficial middle cerebral vein and its anastomosis veins under the operating microscope.
[results]: superficial middle cerebral vein were single type (10 sides), double trunk type (11 cases), three dry type (6 cases) and immature type (3 cases). Anastomotic vein (Trolard vein) and inferior anastomotic vein (Labbe vein) is the main vein anastomosis of superficial cerebral veins. They are contacted with superficial middle cerebral vein. Not all cerebral hemisphere anastomotic veins were developed.Trolard vein in the left hemisphere were absent in 4 cases, 4 cases of right hemisphere of 9 cases of.Labbe vein of left hemisphere absence, 3 cases of right hemisphere.
[Conclusion] in the Department of Neurosurgery surgery, mastering the microsurgical anatomy of the vein is very important to determine the intraoperative venous injury can cause postoperative complications, vein can be preserved to retain as much as possible. Pterional approach for the separation of lateral fissure, should master the micro anatomy of the superficial middle cerebral vein anastomosis in vein. In the case of injury developed superficial middle cerebral vein will cause complications.
